H2H营养管理模式下的持续营养补充对食管癌患者术后营养状况及生活质量的改善作用OA
Continuous Nutritional Supplementation under Hospital-to-Home Nutritional Management Improves Postoperative Nutritional Status and Quality of Life of Esophageal Cancer Patients
为了探讨了 H2H(Hospital To Home)营养管理模式下的持续营养补充对食管癌患者术后营养状况及生活质量的影响,选取了在胸外科首次确诊为食管癌并接受手术治疗的81例患者为研究对象,随机分为实验组和对照组.对照组实行常规营养管理,实验组采取H2H营养管理模式进行持续营养补充.结果显示,两组患者术后血红蛋白浓度均有不同程度下降,且实验组术后3d的血红蛋白浓度低于对照组(P=0.037).术后3d的血清白蛋白(38.2 g·L-1)及前白蛋白(256.7 g·L-1)浓度实验组均高于对照组(36.3、180.9 g·L-1,均有P<0.05).两组患者术后总蛋白和前白蛋白浓度均有下降,出院后逐渐回升,实验组的总体趋势高于对照组.对生活质量进行评估,实验组术后6个月的躯体功能、情绪功能及总体健康水平得分均显著高于对照组(81.7 vs 70.2,85.5 vs 75.0,73.0 vs 62.8,均有P<0.05).综上,H2H营养管理模式下的持续营养补充可改善食管癌患者术后营养状况,提高生活质量,有助于改善预后.H2H营养管理模式可为肿瘤患者术后长期营养管理提供参考模式.
The effect of continuous nutritional supplementation on the postoperative nutritional status and quality of life(QOL)of patients with esophageal cancer under the Hospital-To-Home(H2H)nutritional management model was investigated in this study.A total of 81 patients first diagnosed with esophageal cancer and who had received surgical treatment at the Department of Thoracic Surgery of our hospital were randomly assigned to an experimental or a control group.Routine nutritional management was implemented in the control group,while the H2H nutritional management model was adopted in the experimental group to provide continuous nutritional supplementation.The results showed that postoperative hemoglobin concentration decreased to varying degrees in both groups;however,the experimental group had a lower concentration at 3 days postoperatively than the control group(P=0.037).The concentrations of serum albumin(38.2 g·L-1)and prealbumin(256.7 g·L-1)at 3 days postoperatively were higher in the experimental than in the control group(36.3 and 180.9 gL-1,respectively;both P<0.05).In both groups,total protein and prealbumin concentrations decreased postoperatively,followed by a gradual increase after discharge,with the experimental group showing a steeper overall trend than the control group.QOL assessment indicated that the experimental group achieved significantly higher scores in physical functioning(81.7 vs 70.2),emotional functioning(85.5 vs 75.0),and overall health(73.0 vs 62.8)at 6 months postoperatively than the control group(all P<0.05).In conclusion,the results of this study indicate that continuous nutritional supplementation based on the H2H nutritional management model simultaneously enhances postoperative nutritional status and QOL in esophageal cancer patients,thereby improving postoperative outcomes.The H2H nutritional management model can serve as a reference for long-term nutritional management of postoperative cancer patients.
