结直肠癌术后病人居家自我管理问题提示清单的构建与应用OA
Development and application of a question prompt list for home-based self-management in postoperative patients with colorectal cancer
目的:构建适用于结直肠癌术后病人居家自我管理的问题提示清单,并验证其临床可行性.方法:采用文献回顾、半结构式访谈与Delphi专家函询相结合的方法,收集结直肠癌术后病人居家自我管理核心问题,形成结直肠癌术后病人居家自我管理的问题提示清单.采用便利抽样法,选取2025年5月—7月出院的结直肠癌术后病人为研究对象,从内容合理性、表达通俗性、使用便捷性、教育作用和沟通促进5个维度验证问题提示清单的临床可行性.结果:结直肠癌术后病人居家自我管理的问题提示清单包括9个一级指标、69个二级指标,涵盖疾病知识、治疗信息、症状管理、营养管理、运动管理、造口管理、心理支持、社会支持及随访复查.2轮函询专家权威系数分别为0.912和0.896,肯德尔协调系数分别为0.296,0.318(均P<0.001).病人内容合理性、表达通俗性、使用便捷性、教育作用和沟通促进5个维度的可行性分别为92.0%、94.0%、88.0%、86.0%、76.0%.结论:构建的结直肠癌术后病人居家自我管理问题提示清单科学性强、结构完整、通俗易用,可帮助结直肠癌术后病人居家阶段识别问题、表达需求与自我管理,为数字化健康教育工具的研发提供依据.
Objective:To develop a question prompt list for home-based self-management of postoperative patients with colorectal cancer,and to evaluate its clinical feasibility.Methods:A mixed-methods approach combining literature review,semi-structured interviews,and Delphi expert consultation were adopted to identify core self-management issues related to home-based self-management in postoperative patients with colorectal cancer.A question prompt list for home-based self-management in postoperative patients with colorectal cancer was developed.Using convenience sampling method,postoperative patients with colorectal cancer discharged from May to July 2025 were selected as the research subjects.The clinical feasibility of the question prompt list was verified from five dimensions:content adequacy,clarity of expression,ease of use,educational value,and communication facilitation.Results:The Question Prompt List for Home-based Self-management in Postoperative Patients with Colorectal Cancer included 9 primary indicators and 69 secondary indicators,covering disease knowledge,treatment information,symptoms management,nutrition management,physical activity management,stoma management,psychological support,social support,and follow-up review.The authoritative coefficients of the two rounds of inquiry were 0.912 and 0.896,respectively.Kendall's coordination coefficients of the two rounds of inquiry were 0.296 and 0.318,respectively(both P<0.001).The feasibility of the five dimensions of patient content adequacy,clarity of expression,ease of use,educational value,and communication facilitation were 92.0%,94.0%,88.0%,86.0%,and 76.0%,respectively.Conclusions:The Question Prompt List for Home-based Self-management in Postoperative Patients with Colorectal Cancer constructed in this study has strong scientific validity,complete structure,and is easy to use.It could help the identification of problems,expression of needs,and self-management during the home stage of patients with postoperative colorectal cancer,and provide basis for the development of digital health education tools.
