加味四妙散治疗宫颈炎伴HPV感染的疗效及对患者阴道微生态、远期预后的影响OA
Effect of Jiawei Simiao powder on cervicitis associated with HPV infection and its influence on vaginal microecology and long-term prognosis
目的:分析加味四妙散治疗宫颈炎伴人乳头瘤病毒(HPV)感染的疗效及对患者阴道微生态、远期预后的影响.方法:选择120例宫颈炎伴HPV感染患者,分为对照组和观察组,各60例.对照组采取人干扰素α-2b阴道泡腾片治疗,观察组在此基础上,加用加味四妙散治疗.记录两组患者的阴道分泌物清洁度、阴道排液量、结痂止血时间、创面愈合时间,检测治疗前后血清白介素-6(IL-6)、γ-干扰素(IFN-γ)、肿瘤坏死因子-α(TNF-α)、阴道微生态指标(菌群多样性、菌群密集度、阴道pH值),随访6个月,比较两组总有效率、HPV转阴率、不良反应发生率.结果:与对照组比较,观察组阴道分泌物清洁度、阴道排液量均更低,结痂止血时间、创面愈合时间均更短(P＜0.05);与对照组比较,观察组治疗后血清IL-6、TNF-α水平更低,IFN-γ水平更高(P＜0.05);与对照组比较,观察组治疗后菌群多样性、菌群密集度均更优,阴道pH值更小(P＜0.05);与对照组比较,观察组总有效率、HPV转阴率均更高(P＜0.05);两组不良反应比较无统计学差异(P＞0.05).结论:加味四妙散治疗宫颈炎伴HPV感染可进一步提高疗效,有助于改善阴道微生态和远期预后,具有一定临床应用价值.
Objective:To analyze the curative effect of Jiawei Simiao powder on cervicitis associated with human papillomavirus(HPV)infection and its influence on vaginal microecology and long-term prognosis.Methods:A total of 120 patients with cervicitis complicated by HPV infection were selected and divided into a control group and an ob-servation group,with 60 cases in each group.The control group was treated with human interferon α-2b vaginal effer-vescent tablets,the observation group based on this,and with Jiawei Simiao powder treatment.The cleanliness of va-ginal discharge,vaginal discharge volume,scab stopping time,and wound healing time of patients in the two groups were recorded.Serum interleukin-6(IL-6),interferon-γ(IFN-γ),tumor necrosis factor-α(TNF-α),vaginal microeco-logical indicators(diversity of bacterial flora,density of bacterial flora,vaginal pH value)were detected before and af-ter treatment.The total effective rate,HPV negative conversion rate and adverse reactions were compared between the two groups after 6 months of follow-up.Results:Compared with the control group,the vaginal secretion cleanli-ness and vaginal discharge volume of the observation group were lower,scab stopping bleeding time and wound heal-ing time were shorter(P＜0.05).Compared with the control group,the levels of IL-6,TNF-α in the observation group after treatment were lower,and the level of IFN-γ was higher,the differences were significant(P＜0.05).Compared with the control group,the diversity and density of microflora in the observation group were better after treatment,and the vaginal pH value was smaller,with significant differences(P＜0.05).Compared with the control group,the total effective rate and HPV negative conversion rate of the observation group were higher,the differences were significant(P＜0.05).No difference was observed in adverse reactions between the two groups(P＞0.05).Conclusion:Jiawei Simiao powder in the treatment of cervicitis with HPV infection can further improve the curative effect,help to improve the vaginal microecology and long-term prognosis,and has a certain clinical promotion value.
