成人脓毒症患者静脉血栓栓塞发生率及影响因素的meta分析OA
Meta-analysis on the incidence and influencing factors of venous thromboembolism in adult patients with sepsis
目的 系统评价成人脓毒症患者静脉血栓栓塞(VTE)发生率及影响因素.方法 计算机检索中国知网、万方、维普、中国生物医学文献服务系统(SinoMed)、PubMed、Web of Science、CINAHL、Embase及Cochrane Library数据库中关于成人脓毒症患者VTE发生率及影响因素的文献,检索时限为建库至2025年10月.完成筛选文献、提取资料和评价文献质量后,采用RevMan5.4软件进行meta分析.结果 纳入17篇文献,其中13篇中文文献,4篇英文文献.meta分析结果显示,脓毒症患者 VTE的总体发生率为24%(95%CI:16%~31%).纳入研究两篇以上、进行有效数据分析的影响因素共有18个,包括患者一般因素4个、临床监测指标8个、疾病治疗因素6个.meta分析结果显示,年龄、手术史、D-二聚体、纤维蛋白原、下肢功能障碍、机械通气是成人脓毒症患者发生VTE的影响因素(P<0.05).结论 临床医护人员应加强对脓毒症VTE高危因素的早期识别,并积极采取针对性干预措施,以有效降低VTE的发生风险.
Objective To systematically evaluate the incidence and influencing factors of venous throm-boembolism(VTE)in adult patients with sepsis.Methods Databases including CNKI,Wanfang,VIP,Si-noMed,PubMed,Web of Science,CINAHL,Embase,and Cochrane Library were systematically searched for studies reporting the incidence and influencing factors of VTE in adult patients with sepsis from inception to October 2025.After screening,data extraction,and quality assessment,a meta-analysis was performed using RevMan 5.4 soft ware.Results A total of 17 studies were included,comprising 13 Chinese and 4 English studies.Meta-analysis showed that the overall incidence of VTE in patients with sepsis was 24%(95%CI:16%-31%).A total of 18 influencing factors were analyzed(with data from≥2 studies),including 4 general patient factors,8 clinical monitoring indicators,and 6 disease treatment-related factors.Meta-analysis revealed that age,history of surgery,D-dimer,fibrinogen,lower extremity dysfunction,and mechanical ventilation were influencing factors for VTE in adult patients with sepsis(P<0.05).Conclusion Clinical healthcare providers should strengthen early identification of high-risk factors for VTE in sepsis and implement targeted interven-tions to effectively reduce the risk of VTE.
