胃肠道肿瘤加速康复外科质量评价指标体系的构建OACHSSCD
Development of a quality evaluation index system for enhanced recovery after surgery in gastrointestinal tumors
目的 构建适用于胃肠道肿瘤加速康复外科(ERAS)的评价指标体系,为临床提供科学、系统的质量评价工具.方法 在Donabedian"结构-过程-结果"质量理论指导下,通过文献回顾和小组讨论初步拟定指标体系框架,采用Delphi法对30名来自胃肠外科、麻醉科、营养科及护理管理领域的专家进行两轮函询,对指标进行筛选和论证.结果 专家函询问卷第一轮回收率为93.8％,第二轮回收率为100％,专家权威系数为0.92,三级指标肯德尔协调系数由0.241提升至0.283(P＜0.05).最终构建的指标体系包括3个一级指标、10个二级指标、36个三级指标.结论 本研究构建的胃肠道肿瘤ERAS评价指标体系专家积极性高、权威程度好、意见协调性强,指标体系科学可靠,初步可为胃肠道肿瘤ERAS的临床实践和质量控制提供标准化评价工具.
Objective To construct an evaluation index system for enhanced recovery after surgery(ERAS)in gastrointestinal tumors,providing a scientific and systematic quality assessment tool for clinical practice.Methods Guided by Donabedian's"structure-process-outcome"quality theory,a preliminary framework of the evaluation index system was developed through literature review and panel discussion.A two-round Delphi method was conducted among 30 experts from the fields of gastrointestinal surgery,anesthesiology,nutrition,and nursing management to screen and validate the indicators.Results The response rates for the two rounds of surveys were 93.8％and 100.0％,respectively,with an expert authority coefficient of 0.92.The Kendall's coefficient of concordance for tertiary indicators increased from 0.241 to 0.283(P＜0.05).The final evaluation index system consisted of 3 primary indicators,10 secondary indicators,and 35 tertiary indicators.Conclusion The established ERAS evaluation index system for gastrointestinal tumors demonstrates high expert enthusiasm,strong authority,and good consensus.It is scientifically reliable and can serve as a preliminary standardized quality assessment tool for ERAS clinical practice and quality control in gastrointestinal tumors.
