基于"浊毒"理论探究慢性胰腺炎的病机与证治OA

Chronic pancreatitis(CP)is a chronic and progressive inflammatory disease of the pancreatic tissue,classified under the category of"abdominal pain"in traditional Chinese medicine(TCM).Its core pathogenesis lies in"spleen deficiency impairing transportation and transformation,with turbid toxin accumulating internally".The theory of"turbid toxin"originated from the Huangdi Neijing and evolved from the foundational concept of"clear and turbid".Drawing on historical insights into the nature of"toxin",this theory was further expanded,with"turbidity"originally referring to dampness and"toxin"to the accumulation of pathogenic factors.The two intertwine to form a sticky,filthy pathological product.This study analyzes CP's pathogenesis from the theoretical origins and implications of"turbid toxin":Spleen and stomach deficiency leads to impaired transportation and transformation,disrupting the distribution of essential nutrients and giving rise to internal dampness.Prolonged stagnation transforms dampness into heat and eventually into toxin.The mutual accretion of turbidity and toxin results in pancreatic tissue damage and dysfunction of both exocrine and endocrine functions.Therefore,treatment should address both the root and the branch,primarily by tonifying Qi and elevating Yang,while also eliminating turbid toxin and resolving accumulation.Clinical evidence supports the efficacy of the"Qi-tonifying and turbid toxin-resolving"approach in managing CP.This study explores the TCM pathogenesis,syndrome differentiation,and treatment of CP based on the theory of turbid toxin,providing a theoretical foundation for its clinical application.
