目的 观察清热逐风合剂对类风湿关节炎(rheumatoid arthritis,RA)模型——胶原诱导型关节炎(CIA)大鼠血清白细胞介素 17 A(IL-17A)、转化生长因子β(TGF-β)及滑膜组织缺氧诱导因子 1α(HIF-1α)、血管内皮细胞生长因子(VEGF)表达的影响,探讨清热逐风合剂缓解类风湿关节炎大鼠炎症反应的作用机制.方法 建立CIA大鼠模型,分别以甲氨蝶呤片、雷公藤多苷片和清热逐风合剂作为干预因素,观测各组大鼠足趾肿胀度、关节炎指数变化,苏木精-伊红染色法(HE)检测大鼠滑膜组织病理改变,酶联免疫吸附法(ELISA)检测外周血IL-17A和TGF-β的表达,蛋白免疫印迹法(Western Blot)法检测HIF-1α、VEGF的表达.结果 与正常组比较,模型组大鼠足趾肿胀度、关节炎指数明显升高(P<0.01);血清IL-17A含量明显升高、TGF-β含量明显降低(P<0.01);滑膜组织HIF-1α、VEGF蛋白表达明显升高(P<0.01).与模型组比较,清热逐风合剂组、甲氨蝶呤片组、雷公藤多苷片组大鼠足趾肿胀度、关节炎指数明显降低(P<0.01);血清IL-17A含量明显降低、TGF-β含量明显升高(P<0.01);滑膜组织中HIF-1α、VEGF蛋白表达明显降低(P<0.01).结论 调控IL-17A、TGF-β之间的比例,使CD4+T细胞向调节性T细胞(Treg)分化,抑制HIF-1α、VEGF表达,促使Th17/Treg趋于平衡,可能是清热逐风合剂缓解类风湿关节炎炎症反应的分子机制之一.

Objective To observe the effects of Qingre Zhufeng Mixture on serum interleukin-17A(IL-17A),transforming growth factor-β(TGF-β),and the expression of hypoxia inducible factor-1α(HIF-1α)and vascular endothelial growth factor(VEGF)in synovial tissue of rats with collagen-induced arthritis(CIA),a model of rheumatoid arthritis(RA),and to explore the mechanism by which Qingre Zhufeng Mixture alleviates the inflammatory response in RA.Methods A CIA rat model was established.Methotrexate tablets,tripterygium glycoside tablets,and Qingre Zhufeng Mixture were used as interventions.Paw swelling and arthritis index were measured.Hematoxylineosin(HE)staining was used to observe histopathological changes in synovial tissue.Serum levels of IL-17A and TGF-β were detected by enzymelinked immunosorbent assay(ELISA).The protein expression of HIF-1α and VEGF in synovial tissue was detected by Western Blot.Results Compared with the normal group,the model group showed significantly increased paw swelling and arthritis index(P<0.01);significantly elevated serum IL-17A level and decreased serum TGF-β level(P<0.01);and significantly increased protein expression of HIF-1α and VEGF in synovial tissue(P<0.01).Compared with the model group,the Qingre Zhufeng Mixture group,methotrexate group,and tripterygium glycoside group all showed significantly reduced paw swelling and arthritis index(P<0.01);significantly decreased serum IL-17A and increased serum TGF-β(P<0.01);and significantly reduced protein expression of HIF-1α and VEGF in synovial tissue(P<0.01).Conclusion Regulating the IL-17A/TGF-β balance to promote CD4+T cell differentiation toward regulatory T cells(Tregs),inhibiting the expression of HIF-1α and VEGF,and thereby restoring the Th17/Treg balance may be one of the molecular mechanisms by which Qingre Zhufeng Mixture alleviates the inflammatory response in rheumatoid arthritis.
