目的 基于EphrinB2/Eph受体B4(EphB4)/胞外信号调节蛋白激酶(ERK)通路探讨补骨生髓方改善去势大鼠骨丢失的作用机制.方法 将雄性SD大鼠随机分为假手术组、模型组、阿仑膦酸钠组(0.88 mg·kg-1)及补骨生髓方低、中、高剂量组(2.94、5.87、11.74 g·kg-1),每组 15 只.除假手术组外,其余各组行双侧睾丸切除术,复制去势性骨质疏松大鼠模型.术后第 2 天开始给药干预,各给药组按照上述剂量给予灌胃,假手术组、模型组给予等量蒸馏水灌胃,每日 1 次,连续灌胃 6 d后休息 1 d,共给药 12 周.采用Micro-CT检测大鼠股骨松质骨的三维微结构参数及骨矿物质密度,指标包括:骨体积分数(BV/TV)、骨小梁厚度(Tb.Th)、骨小梁数(Tb.N)、骨小梁分离度(Tb.Sp)、结构模型指数(SMI)及骨矿物质密度(BMD);HE染色法观察骨组织病理形态变化;采用全自动生化仪检测血清中丙氨酸氨基转移酶(ALT)、天门冬氨酸氨基转移酶(AST)、尿素氮(BUN)、肌酐(CREA)水平;ELISA法检测血清骨Gla蛋白(BGP)、骨碱性磷酸酶(BALP)水平;RT-qPCR法检测骨组织ALP、骨钙素(OCN)mRNA表达水平;免疫组化法检测骨组织Runt相关转录因子 2(RUNX2)蛋白表达水平;Western Blot法检测骨组织EphrinB2、EphB4、p-EphB4、ERK蛋白表达水平.结果 与假手术组比较,模型组股骨远端BMD、BV/TV、Tb.N显著降低(P<0.01),Tb.Sp、SMI显著升高(P<0.01);血清BALP、BGP水平显著降低(P<0.01);骨组织ALP、OCN mRNA表达显著下调(P<0.01);骨组织EphrinB2、EphB4、p-EphB4、ERK、RUNX2 蛋白表达及p-EphB4/EphB4 比值均显著下调(P<0.01),RUNX2 平均光密度值显著降低(P<0.01).与模型组比较,阿仑膦酸钠组及补骨生髓方中、高剂量组大鼠股骨远端BMD、BV/TV、Tb.N显著升高(P<0.01),Tb.Sp、SMI显著降低(P<0.05,P<0.01),血清BALP、BGP水平显著升高(P<0.05,P<0.01);补骨生髓方中剂量组大鼠股骨远端Tb.Th明显增加(P<0.05),骨组织ALP、OCN mRNA表达显著上调(P<0.01),骨组织EphrinB2、EphB4、p-EphB4、ERK、RUNX2 蛋白表达及p-EphB4/EphB4 比值均显著上调(P<0.05,P<0.01),RUNX2 平均光密度值显著升高(P<0.01).各组大鼠的血清 ALT、AST、BUN、CREA水平的差异均无统计学意义(P>0.05).结论 补骨生髓方可改善去势大鼠股骨松质骨三维微结构,提升骨矿物质密度,促进骨形成,发挥骨保护作用,其机制可能与激活EphrinB2/EphB4/ERK信号通路并增强成骨相关因子表达有关.

Objective To investigate the mechanism by which Bugu Shengsui Formula ameliorates bone loss in orchidectomized rats based on the EphrinB2/Eph receptor B4(EphB4)/extracellular signal-regulated kinase(ERK)pathway.Methods Male SD rats were randomly divided into sham operation group,model group,alendronate sodium group(0.88 mg·kg-1),and low-,medium-,and high-dose Bugu Shengsui Formula groups(2.94,5.87,11.74 g·kg-1),with 15 rats per group.Except for the sham operation group,all rats underwent bilateral orchidectomy to establish a castration-induced osteoporosis model.Drug intervention began on day 2 post-surgery.The treatment groups received the corresponding doses by intragastric gavage,while the sham operation group and model group received an equal volume of distilled water.Gavage was performed once daily for 6 consecutive days followed by 1 day of rest,for a total of 12 weeks.Micro-CT was employed to evaluate the three-dimensional trabecular microarchitecture parameters and bone mineral density(BMD)of the distal femur,including bone volume fraction(BV/TV),trabecular thickness(Tb.Th),trabecular number(Tb.N),trabecular separation(Tb.Sp),structure model index(SMI),and BMD.Histopathological changes in bone tissue were observed by HE staining.Serum levels of alanine aminotransferase(ALT),aspartate aminotransferase(AST),blood urea nitrogen(BUN),and creatinine(CREA)were measured using an automatic biochemical analyzer.Serum bone Gla protein(BGP)and bone alkaline phosphatase(BALP)levels were determined by ELISA.RT-qPCR was performed to detect the mRNA expression levels of ALP and osteocalcin(OCN)in bone tissue.Immunohistochemistry was used to detect the protein expression of Runt-related transcription factor 2(RUNX2).Western Blot was used to measure the protein expression levels of EphrinB2,EphB4,p-EphB4,and ERK in bone tissue.Results Compared with the sham operation group,the model group exhibited significantly decreased BMD,BV/TV,and Tb.N of the distal femur(P<0.01),significantly increased Tb.Sp and SMI(P<0.01),significantly reduced serum BALP and BGP levels(P<0.01),significantly downregulated mRNA expression of ALP and OCN(P<0.01),and significantly downregulated protein expression of EphrinB2,EphB4,p-EphB4,ERK,and RUNX2,as well as the p-EphB4/EphB4 ratio(P<0.01),accompanied by a significantly decreased mean optical density of RUNX2(P<0.01).Compared with the model group,the alendronate sodium group and the medium-and high-dose Bugu Shengsui Formula groups showed significantly increased BMD,BV/TV,and Tb.N(P<0.01),significantly decreased Tb.Sp and SMI(P<0.05,P<0.01),and significantly elevated serum BALP and BGP levels(P<0.05,P<0.01).In addition,the medium-dose Bugu Shengsui Formula group demonstrated a significant increase in Tb.Th(P<0.05),significantly upregulated ALP and OCN mRNA expression(P<0.01),and significantly upregulated protein expression of EphrinB2,EphB4,p-EphB4,ERK,and RUNX2,as well as the p-EphB4/EphB4 ratio(P<0.05,P<0.01),with a significantly increased mean optical density of RUNX2(P<0.01).No significant differences were found in serum ALT,AST,BUN,or CREA levels among all groups(P>0.05).Conclusion Bugu Shengsui Formula can improve the three-dimensional microarchitecture of trabecular bone in the distal femur,increase bone mineral density,promote bone formation,and exert osteoprotective effects in orchidectomized rats.The mechanism may be associated with activation of the EphrinB2/EphB4/ERK signaling pathway and enhanced expression of osteogenic factors.
