早期肠内营养支持对极低出生体重早产儿体格发育及并发症的影响OA
To explore the effect of early enteral nutrition support on physical development and complications in very low birth weight infants
目的 探讨早期肠内营养支持对极低出生体重儿体格发育及并发症的影响.方法 选取2023年6月1日至2025年6月30日青海大学附属医院新生儿重症监护医学科和青海省妇女儿童医院新生儿科收治的156例极低出生体重儿(very low birth weight infants,VLBWI)作为研究对象.根据2024年7月开始实施早产儿标准化肠内喂养方案为界,实施方案前纳入对象为对照组(2023年6月1日至2024年5月31日,n=82)和实施方案后纳入对象为试验组(2024年7月1日至2025年6月30日,n=74).比较两组早产儿达全肠道喂养时间、肠外营养时间、住院时间、住院费用、并发症发生率及体格发育指标.结果 试验组早产儿达全肠道喂养时间、肠外营养时间、住院时间及住院费用均显著低于对照组(P<0.05).试验组并发症总发生率较对照组降低,但差异无统计学意义(P>0.05).与对照组比较,试验组早产儿生后21d、28d的身长、头围、体质量指数增加,差异有统计学意义(P<0.05).结论 对入住新生儿重症监护病房的VLBWI实施早期肠内营养方案可缩短达全肠道喂养时间及住院时间;该方案未显著增加喂养不耐受、坏死性小肠结肠炎风险,且可能有助于后期追赶生长,但短期效果不明显.
Objective To investigate the effects of early enteral nutritional support on physical development and complications in very low birth weight infants(VLBWI).Methods A total of 156 VLBWI admitted to Department of Neonatal Intensive Care Unit,Qinghai University Affiliated Hospital and Department of Neonatology,Women and Children's Hospital of Qinghai Province between June 1,2023,and June 30,2025,were included in the study.Using the implementation of the standardized enteral feeding protocol for preterm infants on July 1,2024,as the cutoffpoint,infants enrolled before the implementation were assigned to control group(June 1,2023,to May 31,2024;n=82),and those enrolled after the implementation were assigned to experimental group(July 1,2024,to June 30,2025;n=74).The time to achieve full enteral feeding,duration of parenteral nutrition,length of hospital stay,hospitalization costs,incidence of complications,and physical growth indicators were compared between two groups of preterm infants.Results In experimental group,the time to achieve full enteral feeding,duration of parenteral nutrition,length of hospital stay,and hospitalization costs were all significantly lower than those in control group(P<0.05).The overall incidence of complications in experimental group was lower than that in control group,but the difference was not statistically significant(P>0.05).Compared with control group,preterm infants in experimental group showed statistically significant increases in length,head circumference,and body mass index at 21 and 28 days after birth(P<0.05).Conclusion Implementing an early enteral nutrition protocol for VLBWI admitted to the neonatal intensive care unit can shorten the time to full enteral feeding and the length of hospital stay;This protocol does not significantly increase the risk of feeding intolerance or necrotizing enterocolitis,and may contribute to catch-up growth later on,although the short-term effects are not pronounced.
