目的 探讨二维超声图像特征联合剪切波弹性成像(shear wave elastography,SWE)参数对预测甲状腺乳头状癌(papillary thyroid carcinoma,PTC)B-Raf 原癌基因丝氨酸/苏氨酸激酶的第600位缬氨酸突变为谷氨酸(B-Raf proto-oncogene,serine/threonine kinase,Val600Glu,BRAFV600E)的应用价值.方法 选取2024年12月至2025年5月于邢台市人民医院接受甲状腺超声检查且术后获得BRAF V600E基因检测结果的120例PTC患者作为研究对象.分析患者的二维超声与SWE特征,采用L1正则化逻辑回归进行特征筛选,并构建二维超声模型、弹性成像模型及联合模型.通过机器学习算法评估模型性能.结果 联合模型的多层感知器算法下受试者操作特征曲线下面积达0.902,显著优于单模态模型.BRAF突变组结节实性比例高、纵横比>1、不规则边缘、微钙化及剪切波弹性成像弹性质量指数值升高(P<0.05).结论 结合二维超声与SWE参数可显著提高PTC术前BRAFV600E突变预测能力,为个体化诊疗提供参考.

Objective To investigate the clinical utility of combining two-dimensional ultrasound image features with shear wave elastography (SWE) parameters in predicting mutations of the B-Raf proto-oncogene,serine/threonine kinase,Val600Glu(BRAFV600E) in patients with papillary thyroid carcinoma (PTC). Methods A total of 120 patients with PTC who underwent thyroid ultrasound examination at Xingtai People's Hospital from December 2024 to May 2025 and obtained postoperative BRAF V600E genetic testing results were selected as study subjects. The two-dimensional ultrasound features and SWE characteristics were analyzed,feature screening was performed using L1-regularized Logistic regression,and two-dimensional ultrasound models,elastic imaging models,and combined models were constructed. Model performance was evaluated employing machine learning algorithms. Results Under the multi-layer perceptron algorithm,the area under the curve of the receiver operating characteristics in the joint model reached 0.902,significantly outperforming that of the single-modal model. The BRAF mutation group exhibited a higher proportion of solid nodules,a long-to-wide ratio>1,irregular margins,microcalcifications,and elevated elastographic quality index values (P<0.05). Conclusion The combination of conventional ultrasound features and SWE parameters can significantly improve the preoperative predictive performance for BRAFV600E mutation in PTC,providing a reference for personalized diagnosis and treatment.
