孕晚期血脂异常对子代6~8岁儿童肥胖的预测价值研究OA
Study on the predictive value of dyslipidemia in late pregnancy for obesity in children aged 6-8 years
目的 探讨孕晚期血脂异常与子代儿童6~8岁肥胖的关联性.方法 选取2014年1月至2016年12月宁波大学附属妇女儿童医院收治的808名孕产妇作为研究对象.根据孕晚期血脂是否异常分为血脂异常组(n=417)和血脂正常组(n=391).收集孕产妇分娩前空腹血清总胆固醇(total cholesterol,TC)、甘油三酯(triglycerides,TG)、高密度脂蛋白胆固醇(high-density lipoprotein cholesterol,HDL-C)和低密度脂蛋白胆固醇(low-density lipoprotein cholesterol,LDL-C)的实验室数据.通过电话随访获取子代身高和体质量信息.采用单因素分析和二分类Logistic回归分析筛选肥胖的影响因素,采用受试者操作特征曲线(receiver operating characteristic curve,ROC曲线)评估预测效能.结果 孕晚期TC和LDL-C异常孕产妇子代肥胖发生率高于正常孕产妇(P<0.05).二分类Logistic回归分析结果表明,TC和LDL-C是子代肥胖的独立危险因素.ROC曲线分析表明,LDL-C、TC及两者联合的曲线下面积分别为0.595、0.648和0.656.TC和LDL-C联合预测的约登指数最大为0.068.结论 孕晚期TC和LDL-C水平升高与子代6~8岁儿童肥胖相关,尤其在高阴性预测值方面具有潜在的分层筛查意义.
Objective To investigate the association between maternal dyslipidemia late pregnancy and obesity in offspring aged 6-8 years.Methods A total of 808 pregnant women admitted to Ningbo University Affiliated Women and Children's Hospital from January 2014 to December 2016 were enrolled as study subjects.They were divided into abnormal lipid profile group(n=417)and normal lipid profile group(n=391)based on the presence or absence of dyslipidemia during late pregnancy.Laboratory data including prepartum fasting serum levels of total cholesterol(TC),triglycerides(TG),high-density lipoprotein cholesterol(HDL-C),and low-density lipoprotein cholesterol(LDL-C)were collected.Information on offspring height and body weight was obtained through telephone follow-up.Univariate analysis and binary Logistic regression analysis were employed to identify factors influencing obesity,while the receiver operating characteristic(ROC)curve was used to evaluate predictive performance.Results Abnormal levels of TC and LDL-C during late pregnancy were associated with a higher incidence of offspring obesity compared to normal pregnancies(P<0.05).Bivariate Logistic regression analysis demonstrated that TC and LDL-C were independent risk factors for offspring obesity.ROC curve analysis revealed that the area under the curve for LDL-C,TC,and their combination were 0.595,0.648,and 0.656,respectively.The maximum Jordon index predicted by the combined model of TC and LDL-C was 0.068.Conclusion Elevated levels of TC and LDL-C in late pregnancy are associated with obesity in offspring aged 6-8 years,particularly demonstrating potential stratified screening significance in terms of high negative predictive value.
