大鼠肉瘤病毒癌基因(rat sarcoma viral oncogene family,Ras)是消化道肿瘤中最常见的致癌驱动基因,其突变在结直肠癌、胰腺癌和非小细胞肺癌等多种癌症中扮演着关键角色.然而,由于Ras蛋白独特的结构及其信号通路的复杂性,一度被认为是"不可成药"靶点.近年来,KRAS G12C抑制剂的成功获批打破此僵局,推动了针对G12D等其他突变的抑制剂、泛Ras策略及联合疗法的快速发展.然而,治疗耐药仍是临床上时刻面临的挑战.本文系统综述了Ras抑制剂的作用机制与临床研究进展,探讨其在消化道肿瘤中的单药及联合治疗潜力,并对新型变构抑制剂、蛋白降解技术和生物标志物等未来方向进行展望,旨在为优化临床治疗策略提供参考.
The rat sarcoma viral oncogene family(Ras)is among the most common oncogenic drivers in gastrointestinal malignancies,playing a pivotal role in colorectal cance,pancreatic ductal adenocarcinoma,and other cancers.Owing to the unique structural features of Ras proteins and the complexity of their downstream signaling networks,Ras had long been considered an"undruggable"target.In recent years,the successful approval of KRAS G12C inhibitors has broken this paradigm and accelerated the development of inhibitors targeting other mutations such as G12D,as well as pan-Ras strategies and combination therapies.However,therapeutic resistance remains a major clinical challenge.This review summarizes the mechanisms of action and clinical progress of Ras inhibitors,discusses their potential as monotherapy and in combination regimens in gastrointestinal tumors,and highlights future directions,including novel allosteric inhibitors,targeted protein degradation,and biomarker-guided strategies,with the aim of informing optimized clinical treatment.
