晚期非小细胞肺癌患者表皮生长因子受体酪氨酸激酶抑制剂治疗期间症状群特征及对生活质量的影响OA
Characteristics of symptom clusters during epidermal growth factor receptor tyrosine kinase inhibitors treatment in patients with advanced non-small cell lung cancer and their impact on quality of life
目的 分析晚期非小细胞肺癌(non-small cell lung cancer,NSCLC)患者表皮生长因子受体酪氨酸激酶抑制剂(epidermal growth factor receptor tyrosine kinase inhibitor,EGFR-TKI)治疗期间症状群特征及其对生活质量的影响.方法 选取2022年3月至2025年10月于中国人民解放军联勤保障部队第九〇一医院接受EGFR-TKI治疗的98例晚期NSCLC患者为研究对象.使用MD安德森症状量表肺癌特异性模块(MD Anderson symptom inventory-lung cancer,MDASI-LC)和欧洲癌症研究与治疗组织研发的生活质量核心问卷(European Cancer Research and Treatment quality of life questionnaire-core 30,EORTC QLQ-C30)V3.0评估患者治疗期间状群和生活质量特点.采用探索性因子分析(exploratory factor analysis,EFA)提取症状群,并通过多元线性回归分析探讨各症状群对生活质量的影响.结果 患者平均年龄(65.92±8.32)岁,男女比例1:1,多数患者接受第3代EGFR-TKI治疗.患者治疗12周MDASI-LC量表中的疲乏、恶心、睡眠障碍、苦恼、健忘、嗜睡、口干、悲伤、呕吐、麻木和便秘条目评分较基线升高,疼痛、气促、食欲下降、咳嗽条目评分较基线下降(P<0.05).患者治疗12周 EORTC QLQ-C30 V3.0总体健康状况[(63.20±8.64)分比(59.10±9.25)分]、躯体功能、角色功能、社会功能、疼痛、呼吸困难、食欲丧失、便秘评分较基线下降,疲乏、恶心呕吐、失眠、腹泻、经济困难评分较基线升高(P<0.05).提取患者治疗12周MDASI-LC数据进行因子分析[Kaiser-Meyer-Olkin检验(Kaiser-Meyer-Olkin test,KMO)值=0.78,Bartlett P<0.001],共提取4个症状群,包括疲乏-情感症状群(25.10%)、呼吸道症状群(17.85%)、消化道症状群(14.60%)和神经感觉症状群(10.90%),累积方差贡献率68.45%.Pearson相关性分析显示,疲乏-情感症状群、呼吸道症状群、消化道症状群、神经感觉症状群均与患者总体健康、社会功能、角色功能、情绪功能评分呈负相关(P<0.05).以EORTC QLQ-C30 V3.0的各功能领域得分为因变量,症状群因子分为自变量,调整年龄、性别后进行回归分析,结果显示,总体健康主要受疲乏-情感症状群(β=-0.441)影响,社会功能主要受神经感觉症状群(β=-0.271)影响,角色功能主要受疲乏-情感症状群(β=-0.362)影响,情绪功能主要受疲乏-情感症状群(β=-0.571)、皮肤-黏膜症状群(β=-0.216)影响(均P<0.05).结论 晚期NSCLC患者EGFR-TKI治疗期间常出现疲乏-情感症状群、消化道症状群、呼吸道症状群、神经感觉症状群,并伴随着总体健康状况的下降,其中疲乏-情感症状群、消化道症状群对患者生活质量的影响最为显著.
Objective To analyze the characteristics of symptom clusters and their impact on quality of life in patients with advanced non-small cell lung cancer(NSCLC)during treatment with epidermal growth factor receptor tyrosine kinase inhibitors(EGFR-TKI).Method A total of 98 patients with advanced NSCLC who received EGFR-TKI treatment in our hospital from March 2022 to October 2025 were selected as the study subjects.The MD Anderson symptom inventory-lung cancer(MDASI-LC)and the European Cancer Research and Treatment quality of life questionnaire-core 30(EORTC QLQ-C30)V3.0 were used to assess the characteristics of symptom clusters and quality of life during treatment.Exploratory factor analysis(EFA)was used to extract symptom clusters,and multiple linear regression analysis was performed to explore the impact of each symptom cluster on quality of life.Result The mean age of the patients was(65.92±8.32)years,with a male-to-female ratio of 1:1,and most patients received third-generation EGFR-TKI treatment.After 12 weeks of treatment,the scores of fatigue,nausea,sleep disturbance,distress,forgetfulness,drowsiness,dry mouth,sadness,vomiting,numbness,and constipation on the MDASI-LC scale were significantly higher than baseline,while the scores of pain,shortness of breath,decreased appetite,and cough were significantly lower than baseline(all P<0.05).After 12 weeks of treatment,the scores of global health status(63.20±8.64 vs 59.10±9.25),physical function,role function,social function,pain,dyspnea,appetite loss,and constipation on the EORTC QLQ-C30 V3.0 scale were significantly lower than baseline,while the scores of fatigue,nausea and vomiting,insomnia,diarrhea,and financial difficulties were significantly higher than baseline(all P<0.05).Factor analysis was performed on the MDASI-LC data of patients after 12 weeks of treatment(KMO=0.78,Bartlett's test P<0.001),and a total of 4 symptom clusters were extracted,namely fatigue-emotional symptom cluster(25.10%),respiratory symptom cluster(17.85%),gastrointestinal symptom cluster(14.60%),and neurosensory symptom cluster(10.90%),with a cumulative variance contribution rate of 68.45%.Pearson correlation analysis showed that the fatigue-emotional symptom cluster,respiratory symptom cluster,gastrointestinal symptom cluster,and neurosensory symptom cluster were all negatively correlated with the scores of global health,social function,role function,and emotional function in patients(all P<0.05).Regression analysis was conducted with the scores of each functional domain of the EORTC QLQ-C30 as dependent variables and the factor scores of symptom clusters as independent variables,after adjusting for age and gender.The results showed that global health was mainly affected by the fatigue-emotional symptom cluster(β=-0.441),social function was mainly affected by the neurosensory symptom cluster(β=-0.271),role function was mainly affected by the fatigue-emotional symptom cluster(β=-0.362),and emotional function was mainly affected by the fatigue-emotional symptom cluster(β=-0.571)and skin-mucosal symptom cluster(β=-0.216)(all P<0.05).Conclusion Patients with advanced NSCLC frequently experience fatigue-emotional symptom cluster,gastrointestinal symptom cluster,respiratory symptom cluster,and neurosensory symptom cluster during EGFR-TKI treatment,accompanied by a decline in global health status.Among them,the fatigue-emotional symptom cluster and gastrointestinal symptom cluster have the most significant impact on patients'quality of life.
