Research progress of polatuzumab vedotin in the treatment of diffuse large B-cell lymphoma
尽管针对弥漫大B细胞淋巴瘤(DLBCL)的治疗手段持续进步,包括R-CHOP化疗、CAR-T疗法、造血干细胞移植等,但仍有大量患者面临复发或对一线化疗原发耐药,治疗方案的更新迫在眉睫.维泊妥珠单抗是一种靶向CD79b的新型药物,因其在Ⅲ期POLARIX研究中与利妥昔单抗、环磷酰胺、多柔比星和泼尼松联合治疗(Pola-R-CHP)所展现的积极结果而备受关注.本文论述了维泊妥珠单抗的结构、作用机制及其在DLBCL中的临床应用现状,同时对其局限性提出了应对策略,以期为维泊妥珠单抗在DLBCL治疗中发挥更大作用提供理论基础.
Despite continuous advancements in the treatment of diffuse large B-cell lymphoma(DLBCL),including R-CHOP chemotherapy,CAR-T cell therapy,and hematopoietic stem cell transplantation,a substantial proportion of patients still experience relapse or exhibit refractory disease to first-line regimens,underscoring the urgent need for novel therapeu-tic strategies.Polatuzumab vedotin,an antibody-drug conjugate targeting CD79b,has garnered significant attention follow-ing its positive outcomes in the phase Ⅲ POLARIX trial,where it was combined with rituximab,cyclophosphamide,doxoru-bicin,and prednisone(Pola-R-CHP).This review elucidates the structure,mechanism of action,and current clinical appli-cations of polatuzumab vedotin in DLBCL.Furthermore,it addresses the existing limitations of this agent and proposes po-tential countermeasures,aiming to provide a theoretical foundation for optimizing its role in the DLBCL treatment land-scape.
