肠道菌群(GM)失衡通过代谢产物异常持续激活内质网应激(ERS)及未折叠蛋白反应(UPR)信号通路,诱导效应T细胞耗竭、树突状细胞抗原呈递受阻及髓源性抑制细胞免疫抑制表型形成,构建免疫逃逸网络,是非小细胞肺癌免疫治疗应答率差异的重要机制.围绕"GM-ERS-免疫"轴,可将菌群稳态破坏、代谢链路失衡与肿瘤微环境重塑加以整合解释,而中医理论中"上下交损治其中"强调以脾胃为枢纽调和肺肾功能,其内涵与上述菌群-代谢-免疫调控网络相对应,为干预免疫抑制提供了理论支点.建中法可通过重塑菌群结构、提升短链脂肪酸水平、减少异常胆汁酸累积并抑制UPR关键通路的异常激活,改善免疫细胞功能并增强免疫检查点抑制剂(ICI)疗效.未来仍需在菌群代谢谱、UPR核心节点及免疫细胞表型等层面开展系统研究,以明确多靶点协同机制,推动中西医结合的精准干预策略构建,从而提升免疫治疗的整体获益.

Gut microbiota(GM)dysbiosis persistently activates endoplasmic reticulum stress(ERS)and unfolded pro-tein response(UPR)signaling pathways through aberrant microbial metabolites,thereby inducing effector T-cell exhaus-tion,impairing dendritic cell antigen presentation,and promoting the immunosuppressive phenotype of myeloid-derived suppressor cells.This process establishes a coordinated immune evasion network and represents a key mechanism underly-ing heterogeneous responses to immunotherapy in non-small cell lung cancer.The"GM-ERS-immune"axis provides an integrated framework linking microbial homeostasis disruption,metabolic imbalance,and tumor microenvironment remodel-ing.The traditional concept of"Treating the Middle When Upper and Lower Are Injured"emphasizes the pivotal role of the spleen and stomach in harmonizing lung and kidney functions.Its theoretical implications parallel the modern microbiota-metabolism-immunity regulatory network and offer a conceptual basis for targeting tumor-associated immunosuppression.Jianzhong therapy may enhance immunotherapeutic efficacy by reshaping GM composition,increasing short-chain fatty ac-id production,reducing aberrant bile acid accumulation,and suppressing maladaptive activation of key UPR pathways,thereby restoring immune cell function and potentiating immune checkpoint blockade.Future investigations should incorpo-rate comprehensive analyses of microbial metabolomic profiles,core UPR signaling nodes,and immune cell phenotypes to delin-eate multi-target synergistic mechanisms.Such efforts will facilitate the development of precision integrative strategies combin-ing traditional and modern medicine,ultimately improving overall clinical benefit from cancer immunotherapy.
