[Objective]To summarize Professor Lyu Lijiang's clinical experience in treating lumbar intervertebral disc herniation(LDH)based on the principle of"treating both muscles and bones and applying appropriate treatment according to syndrome differentiation".[Methods]By following Professor Lyu's clinical practice,organizing medical records,and consulting ancient Chinese medical literature as well as modern medical theories,this study elaborates Professor Lyu's understanding of the pathogenesis of LDH,explores the diagnostic and therapeutic thinking centered on"treating both muscles and bones and applying appropriate treatment according to syndrome differentiation",and verifies the approach with a clinical case.[Results]Professor Lyu holds that the pathogenesis of LDH is often associated with the imbalance of muscles and bones,deficiency of Zang-Fu organs,and external pathogenic invasion and overstrain.In clinical treatment,a comprehensive system featuring"coordinating internal and external regulation,and addressing both symptoms and root causes"should be established.He advocates the use of lever positioning manipulation to realign muscles and bones and restore mechanical balance,thereby achieving the external treatment goal of"treating both muscles and bones".Practicing exercises strengthen muscles and bones,consolidate therapeutic effects,and follow the health-preserving principle of"combining movement and rest".Based on"applying appropriate treatment according to syndrome differentiation",medicines are administered according to patients'syndrome types to regulate Zang-Fu organs,harmonize Qi and blood,thereby relieving the root cause of insufficient nourishment muscles and bones.The attached proven case was LDH of Qi stagnation and blood stasis type,primarily characterized by muscle and bone imbalance and Qi-blood stasis induced by physical strain.Lever positioning manipulation was applied to relax and realign the muscles and bones,combined with practicing exercises and Chinese herbs to activate blood circulation to remove stasis,regulate Qi and relieve pain.The patient's symptoms were relieved and physical functions were improved,with no recurrence during follow-up.[Conclusion]Professor Lyu's treatment of LDH based on the concept of"treating both muscles and bones and applying appropriate treatment according to syndrome differentiation"has achieved remarkable results,expanding the traditional Chinese medicine diagnostic and therapeutic approaches for LDH and holding important value for learning and promotion.
