薛己天人相应思想指导下的时间治疗疾病观OA
The Time Treatment of Diseases under the Guidance of the Correspondence Between Human Body and Natural Environment of Xue Ji——Take the Gynecological Syndrome Differentiation and Treatment in the Corrections and Annotations to Fine Formulas for Women
[目的]探讨薛己《校注妇人良方》中因时论治妇人疾病的规律方法.[方法]基于《黄帝内经》《伤寒论》《金匮要略》《校注妇人良方》等医籍,研究薛己《校注妇人良方》中"天人相应"理念指导下妇科证治规律,并爰举医术中验案以佐证之.[结果]薛己诊疗妇人疾病时,深植"天人相应"理念,将因时论治思想全面融入实践.针对妇人虚损多需滋养精血的病理特质,倡导因时施补,契合昼夜阴阳消长与四时气血节律;诊断中以时间为核心凭据,通过特定时段的症状表现辨明脏腑虚实与病位归属;方药配伍紧扣四时气化规律与运气学说,服药时间的择取亦精准贴合人体阴阳节律以助药力发挥.所辑验案系统呈现了其以时间节律为诊疗线索的临证思路,清晰展现了时间维度与妇人疾病发生、发展的内在联系.[结论]薛己《校注妇人良方》以天人相应为核心指导,将因时诊病、因时疗病、因时施方、因时用药贯穿妇科诊治全过程,为妇科疾患的治疗开辟了新路径.
[Objective]To explore the rules and methods of treating women's diseases based on time in Corrections and Annotations to Fine Formulas for Women.[Methods]On the basis of Huangdi Neijing,Shanghan Lun,Jingui Yaolue,and Corrections and Annotations to Fine Formulas for Women and other classic Chinese medical works,study the syndrome differentiation and treatment principles under the correspondence between human body and natural environment of Xue Ji's Corrections and Annotations to Fine Formulas for Women.And attached some medical records for verification.[Results]Xue Ji deeply rooted in the concept of correspondence between human body and natural environment in the diagnosis and treatment of women's diseases,and comprehensively integrated the thought of treatment according to time into clinical practice.Regarding the pathological characteristics of women's deficiency and damage,Xue Ji advocated to apply tonic essence and blood according to the time,which is consistent with the fluctuation of Yin and Yang in day and night and the rhythm of Qi and blood in four seasons;the time as the core credentials in the diagnosis,through specific time symptoms discerning deficiency and excess of the zang-fu viscera and the location of the disease;The prescription concerted application is closely related to the Qi transformation of four seasons and the doctrine of five evolutive phases and six climatic factors,and the choice of medication time is also accurately fit to the Yin-Yang rhythm of the human body to help the medicine play its therapeutic effect.The collected verified cases system demonstrated its clinical thinking based on the time rhythm as the clinical clue,which clearly showed the internal connection between the time dimension and the occurrence and development of gynecological diseases.[Conclusion]Xue Ji's Corrections and Annotations to Fine Formulas for Women takes the correspondence between human body and natural environment concept as the core guidance,adhering to the principles of diagnosing,treating,prescription selection,medication selection based on the time,throughout the entire process of gynecological diagnosis and treatment,which opening a new path for the treatment of gynecological diseases.
