目的:比较不同深度电针夹脊穴治疗非酒精性脂肪性肝病(NAFLD)的疗效差异.方法:将72例NAFLD患者采用随机数字表法分为观察组和对照组,每组36例.两组均予基础治疗.观察组于T7~T10夹脊穴直刺1.5寸,对照组于相同穴位直刺0.5寸,均于同侧T7和T10夹脊穴接电针,选用疏密波,频率2 Hz/100 Hz.每次留针30 min,隔日治疗1次,每周治疗3次,休息1 d,持续治疗8周.疗程结束后,比较两组临床疗效;观察受试者肝脏超声影像学特征、中医症状评分、血清肝功能酶学指标以及血脂代谢相关参数的变化.结果:研究过程中观察组脱落2例,对照组脱落3例.观察组肝脏超声影像学分级有效率为88.2%,高于对照组的57.6%(P<0.01).肝脏超声影像分级方面,观察组脂肪肝程度的改善情况优于对照组(P<0.05).中医症状评分方面,观察组评分下降程度较对照组显著(P<0.01).血清肝功能酶学指标方面,观察组丙氨酸氨基转移酶(ALT)和天门冬氨酸氨基转移酶(AST)水平下降较对照组显著(P<0.05);两组在降低γ-谷氨酰转移酶(GGT)水平方面无统计学差异(P>0.05);血脂代谢参数方面,观察组在降低血清总胆固醇(TC)、甘油三酯(TG)和低密度脂蛋白胆固醇(LDL-C)水平方面优于对照组(P<0.05),但在提高高密度脂蛋白胆固醇(HDL-C)水平方面无统计学差异(P>0.05).结论:电针夹脊穴能改善NAFLD患者的肝脏超声影像学分级、血清肝功能酶学指标及血脂代谢参数;针刺深度对疗效有影响,深刺在改善肝脏超声影像学分级,降低中医症状评分、血清肝功能酶学指标(AST和ALT)以及血脂代谢相关参数(TC、TG和LDL-C)方面优于浅刺,而在降低GGT和升高HDL-C水平方面效果相当.

Objective:To compare the therapeutic efficacy of electroacupuncture at Jiaji(EX-B2)points with different needling depths in treating non-alcoholic fatty liver disease(NAFLD). Methods:Seventy-two patients with NAFLD were randomly allocated to an observation group and a control group using a random number table,with 36 cases per group.Both groups received basic intervention.In addition,the observation group received perpendicular needling at Jiaji(EX-B2)points from T7 to T10 with a penetration depth of 1.5 Cun,and the control group received penetrating needling at the same points but with a penetration depth of 0.5 Cun;needles at the T7 and T10 Jiaji(EX-B2)points were connected to electroacupuncture ipsilaterally,with a sparse-dense wave and a frequency of 2 Hz/100 Hz.The needles were retained for 30 min per session,and the treatment was offered once every other day,3 times weekly,followed by 1 d of rest,for a total of 8 weeks.Clinical efficacy was compared between the two groups at the end of treatment;ultrasonographic features of the liver,traditional Chinese medicine(TCM)symptom score,and serum liver function enzymes and lipid metabolism parameters were assessed among participants. Results:During the study,2 participants dropped out of the observation group and 3 from the control group.The effective rate was 88.2%in the observation group based on the hepatic ultrasonographic grading,higher than 57.6%in the control group(P<0.01).The ultrasonographic grading of the liver showed that the observation group improved fatty liver grade more than the control group(P<0.05).Regarding the TCM symptom score,the observation group showed a greater reduction than the control group(P<0.01).In terms of serum liver function enzymes,the levels of alanine transaminase(ALT)and aspartate transaminase(AST)decreased more significantly in the observation group than in the control group(P<0.05);there was no significant difference in the level of γ-glutamyl transferase(GGT)between the two groups(P>0.05).Regarding the lipid metabolism parameters,the observation group was superior to the control group in reducing the levels of serum total cholesterol(TC),triglyceride(TG),and low-density lipoprotein cholesterol(LDL-C)(P<0.05),while the difference was not significant in elevating the level of serum high-density lipoprotein cholesterol(HDL-C)(P>0.05). Conclusion:Electroacupuncture at Jiaji(EX-B2)points can improve hepatic ultrasonographic grading and serum liver function enzymes and lipid metabolism parameters.The depth of needling affects therapeutic efficacy;deep needling outperforms shallow needling in improving hepatic ultrasonographic grading and decreasing the TCM symptom score and serum liver function enzymes(AST and ALT)and lipid metabolism parameters(TC,TG,and LDL-C),while their effects on reducing GGT and elevating HDL-C levels are equivalent.
