迷走神经刺激(vagus nerve stimulation,VNS)作为非药物神经调控策略,近年来在卒中治疗领域受到广泛关注.VNS可通过增强神经可塑性、调节炎症反应、促进血管修复、改善脑血流与代谢稳态,以及影响兴奋性毒性、氧化应激、细胞凋亡与自噬等过程,改善卒中后神经功能.临床前研究显示,侵入性VNS和非侵入性VNS均能减轻卒中相关脑损伤并促进运动功能恢复.临床研究进一步表明,VNS在卒中急性期与慢性期均与运动、认知与感觉功能改善相关,其对卒中后吞咽功能障碍、卒中后抑郁等并发症具有潜在获益,且安全性与耐受性良好.未来需围绕刺激参数优化、个体化策略与多模式联合等开展深入研究,以提高VNS疗效的可预测性并拓展其临床应用.

As a non-pharmacological neuromodulation strategy,vagus nerve stimulation(VNS)has attracted extensive attention in the field of stroke therapy in recent years.VNS can ameliorate neurological dysfunction after stroke by enhancing neuroplasticity,regulating inflammatory responses,promoting vascular repair,improving cerebral blood flow and metabolic homeostasis,as well as modulating excitotoxicity,oxidative stress,apoptosis,autophagy,and other biological processes.Preclinical studies have demonstrated that both invasive and non-invasive VNS can alleviate stroke-related brain injury and promote the recovery of motor function.Clinical trials have further indicated that VNS is associated with the improvement of motor,cognitive and sensory functions in both acute and chronic stages of stroke,and has potential benefits for complications such as post-stroke dysphagia and post-stroke depression,with favorable safety and tolerability profiles.Future research should focus on the optimization of stimulation parameters,individualized strategies and multimodal combination to improve the predictability of VNS efficacy and expand its clinical applications.
