腔内射频消融闭合术、泡沫硬化剂注射联合点式剥脱术治疗下肢静脉曲张的效果OA
Effect of Endovenous Radiofrequency Ablation Closure,Foam Sclerosing Agent Injection Combined with Point Stripping in the Treatment of Lower Extremity Varicose Veins
目的:探讨腔内射频消融闭合术、泡沫硬化剂注射联合点式剥脱术在下肢静脉曲张中的治疗效果.方法:选取蚌埠医科大学第二附属医院 2024 年 4 月—2025 年 11 月收治的 110 例下肢静脉曲张患者,按随机数字表法分为两组,每组 55 例.对照组给予传统大隐静脉高位结扎加抽剥术治疗,观察组给予腔内射频消融闭合术、泡沫硬化剂注射联合点式剥脱术治疗.比较两组手术指标(手术时间、术中出血量)、术后指标(术后 24 h VAS评分、术后下床活动时间、术后住院时间)、治疗效果及并发症发生情况,比较两组术前及术后 1 个月的静脉疾病严重程度[静脉临床严重程度评分(venous clinical severity score,VCSS)]及生活质量[阿伯丁静脉曲张调查问卷(Aberdeen varicose vein questionnaire,AVVQ)].结果:与对照组相比,观察组手术时间较短,术中出血量较少,术后 24 h VAS评分较低,下床活动时间较早,术后住院时间较短(P<0.05).两组术前病情严重程度及生活质量比较,差异无统计学意义(P>0.05);术后 1 个月,与对照组相比,观察组VCSS、AVVQ评分较低(P<0.05).术后 1 个月,观察组治疗总有效率高于对照组,并发症总发生率低于对照组(P<0.05).结论:腔内射频消融闭合术、泡沫硬化剂注射联合点式剥脱术治疗下肢静脉曲张效果良好,可有效减少术中创伤及术后疼痛,促进患者尽快恢复,降低术后并发症发生率,提升患者生活质量.
Objective:To explore the therapeutic effects of endovascular radiofrequency ablation closure,foam sclerosing agent injection combined with point stripping in the treatment of varicose veins of the lower extremities.Method:A total of 110 patients with varicose veins of the lower extremities admitted to the Second Affiliated Hospital of Bengbu Medical University from April 2024 to November 2025 were selected and divided into two groups by random number table method,with 55 cases in each group.The control group was treated with traditional high ligation and stripping of the great saphenous vein,while the observation group was treated with endovascular radiofrequency ablation closure,foam sclerosing agent injection combined with point stripping.The surgical indicators(operation time,intraoperative blood loss),postoperative indicators(VAS score 24 hours after operation,postoperative time to get out of bed and move around,postoperative hospital stay),therapeutic effects and the occurrence of complications were compared between the two groups.The severity of venous diseases[venous clinical severity score(VCSS)]and quality of life[Aberdeen varicose vein questionnaire(AVVQ)]in the two groups before the operation and one month after the operation were compared.Result:Compared with the control group,the observation group had a shorter operation time,less intraoperative blood loss,a lower VAS score at 24 hours after the operation,an earlier time to get out of bed and move around,and a shorter postoperative hospital stay(P<0.05).There were no statistically significant differences in the preoperative severity of the disease and quality of life between the two groups(P>0.05).One month after the operation,compared with the control group,the VCSS and AVVQ score of the observation group were lower(P<0.05).One month after the operation,the total effective rate of treatment in the observation group was higher than that in the control group,and the total incidence of complications was lower than that in the control group(P<0.05).Conclusion:Endovascular radiofrequency ablation closure,foam sclerosing agent injection combined with point stripping have good therapeutic effects on varicose veins of the lower extremities.They can effectively reduce intraoperative trauma and postoperative pain,promote the patient's recovery as soon as possible,lower the incidence of postoperative complications,and improve the patient's quality of life.
