益肝复明汤加减联合耳穴压豆治疗原发性开角型青光眼的临床效果OA
Clinical Effect of Modified Yigan Fuming Decoction Combined with Auricular Point Pressing with Beans in the Treatment of Primary Open-angle Glaucoma
目的:探讨益肝复明汤加减联合耳穴压豆治疗原发性开角型青光眼的临床效果及安全性.方法:选取 2024 年 1 月—2025 年 8 月于抚州市第一人民医院就诊的原发性开角型青光眼患者 82 例作为研究对象,采用随机数字表法分为对照组和观察组,各 41 例.对照组给予常规西医降眼压治疗,观察组在对照组基础上应用益肝复明汤加减联合耳穴压豆治疗,两组均连续治疗 3 个月.比较两组临床疗效、视力水平、视功能损害指标、光学相干断层扫描(optical coherence tomography,OCT)结构参数、血流动力学指标及安全性.结果:观察组的总有效率高于对照组(P<0.05).治疗后,观察组最佳矫正视力(best corrected visual acuity,BCVA)优于对照组(P<0.05).治疗后,观察组视野平均缺损(mean deviation,MD)低于对照组,视野指数(visual field index,VFI)高于对照组(P<0.05).治疗后,观察组垂直C/D比率、视杯体积低于对照组,盘沿面积大于对照组(P<0.05).治疗后,观察组视网膜中央动脉(central retinal artery,CRA)与睫状后动脉(posterior ciliary artery,PCA)的收缩期峰值流速(peak systolic velocity,PSV)及舒张末期流速(end-diastolic velocity,EDV)高于对照组,而CRA与PCA的阻力指数(resistance index,RI)低于对照组(P<0.05).两组不良反应总发生率比较差异无统计学意义(P>0.05).结论:益肝复明汤加减联合耳穴压豆在常规治疗基础上可进一步改善原发性开角型青光眼患者的视功能表现、视神经结构稳定性及眼部血流动力学状态,延缓视神经损害进展.
Objective:To investigate the clinical efficacy and safety of modified Yigan Fuming Decoction combined with auricular point pressing with beans in the treatment of primary open-angle glaucoma.Method:A total of 82 patients with primary open-angle glaucoma who visited the First People's Hospital of Fuzhou City from January 2024 to August 2025 were selected as the research subjects.The patients were divided into the control group and the observation group by random number table method,with 41 cases in each group.The control group received conventional western intraocular pressure-lowering therapy.The observation group was treated with modified Yigan Fuming Decoction combined with auricular point pressing with beans on the basis of the control group.Both groups were treated continuously for 3 months.Clinical efficacy,vision level,indicators of visual function impairment,optical coherence tomography(OCT)structural parameters,hemodynamic indicators and safety of the two groups were compared.Result:The total effective rate in the observation group was higher than that in the control group(P<0.05).After treatment,the best-corrected visual acuity(BCVA)in the observation group was better than that in the control group(P<0.05).After treatment,the mean deviation(MD)in the observation group was lower than that in the control group,and the visual field index(VFI)was higher than that in the control group(P<0.05).After treatment,the vertical C/D ratio and optic cup volume in the observation group were lower than those in the control group,while the rim area was bigger than that in the control group(P<0.05).After treatment,the peak systolic velocity(PSV)and end-diastolic velocity(EDV)of the central retinal artery(CRA)and posterior ciliary artery(PCA)in the observation group were higher than those in the control group,while the resistance index(RI)of CRA and PCA in the observation group were lower than those in the control group(P<0.05).There was no statistically significant difference in the total incidence of adverse reactions between the two groups(P>0.05).Conclusion:Modified Yigan Fuming Decoction combined with auricular point pressing with beans further improves visual function performance,stabilizes optic nerve structure,and enhances ocular hemodynamics in patients with primary open-angle glaucoma when added to conventional therapy,and delay the progression of optic nerve damage.
