目的:对比股骨近端仿生髓内钉(PFBN)与二代股骨近端抗旋髓内钉(PFNA-Ⅱ)在老年不稳定型股骨粗隆间骨折中的临床应用效果.方法:纳入于共青城市人民医院 2024 年 11 月—2025 年5 月就诊的 68 例老年不稳定型股骨粗隆间骨折患者,经随机数字表法分为两组,每组 34 例.对照组行PFNA-Ⅱ固定,观察组则采用PFBN固定.比较两组围手术期指标、术后恢复指标,术后 1、3、6 个月的Harris评分与VAS评分,术后 6 个月的Barthel指数与Holden步行能力分级,以及术后并发症发生率.结果:观察组术中出血量及透视次数多于对照组,手术时间长于对照组(P<0.05).两组住院时间、骨折愈合时间及术后 1、6 个月的VAS评分比较,差异无统计学意义(P>0.05).观察组首次下地行走时间及完全负重时间早于对照组(P<0.05).观察组术后 3 个月的VAS评分低于对照组,术后 1、3、6 个月的Harris评分高于对照组(P<0.05).术后 6 个月,观察组的Barthel指数、Holden步行能力分级评分高于对照组(P<0.05).观察组术后并发症总发生率(5.9%)与对照组(17.6%)比较,差异无统计学意义(P>0.05).结论:PFBN可有效加速老年不稳定型股骨粗隆间骨折患者术后早期负重,并促进功能恢复,显著改善髋关节功能及生活活动能力.

Objective:To compare the clinical application effects of proximal femoral bionic nail(PFBN)and proximal femoral nail antirotation-Ⅱ(PFNA-Ⅱ)in the treatment of unstable intertrochanteric femoral fractures in the elderly.Method:Sixty-eight elderly patients with unstable intertrochanteric femoral fractures who visited Gongqingcheng People's Hospital from November 2024 to May 2025 were included.They were divided into two groups using a random number table method,with 34 cases in each group.The control group underwent fixation with PFNA-Ⅱ,while the observation group underwent fixation with PFBN.The perioperative indicators,postoperative recovery indicators,Harris scores and VAS scores 1,3,and 6 months after surgery,Barthel index and Holden walking ability grading 6 months after surgery,as well as the incidence of postoperative complications were compared between the two groups.Result:The intraoperative blood loss and the number of fluoroscopy of the observation group were more than those of the control group,the operation time was longer than that of the control group(P<0.05).There were no statistically significant differences in VAS scores 1 and 6 months after surgery,hospital stay,and fracture healing time between the two groups(P>0.05).The time for the first walking after surgery and the time for full weight-bearing in the observation group were earlier than those in the control group(P<0.05).The VAS score of the observation group 3 months after surgery was lower than that of the control group,and the Harris scores 1,3,and 6 months after surgery were higher than those of the control group(P<0.05).At 6 months after surgery,the Barthel index and Holden walking ability grading scores of the observation group were higher than those of the control group(P<0.05).The total incidence of postoperative complications in the observation group(5.9%)was not statistically different from that in the control group(17.6%)(P>0.05).Conclusion:PFBN can effectively accelerate the early weight-bearing of elderly patients with unstable intertrochanteric femoral fractures after surgery,promote functional recovery,and significantly improve hip joint function and daily living ability.
