中西医结合疗法治疗维持性血液透析患者EPO抵抗(脾肾阳虚证)的效果OA
The Effect of Integrated Traditional Chinese and Western Medicine Therapy on Maintenance Hemodialysis Patients with EPO Resistance(Spleen and Kidney Yang Deficiency Syndrome)
目的:探讨中西医结合疗法治疗维持性血液透析(MHD)患者促红细胞生成素(EPO)抵抗(脾肾阳虚证)的临床效果.方法:前瞻性选取 2022 年 2 月—2025 年 2 月赣州市立医院收治的 70 例存在EPO抵抗(脾肾阳虚证)的MHD患者,根据随机数字表法分为对照组(n=35)、观察组(n=35).对照组采用EPO联合多糖铁复合物治疗,观察组采用中西医结合疗法(罗沙司他、多糖铁复合物、生血宁片)治疗,疗程均为 3 个月.对比两组的临床疗效、贫血指标、中医证候积分、炎症因子水平、铁代谢指标及不良反应发生率.结果:治疗后,观察组总有效率高于对照组(P<0.05);治疗后,观察组中医证候积分的主症、次症积分及总分均低于对照组,血红蛋白、血细胞比容、血清铁、转铁蛋白饱和度、铁蛋白水平高于对照组,核因子-κB、γ干扰素水平低于对照组(P<0.05).两组不良反应发生率比较,差异无统计学意义(P>0.05).结论:存在EPO抵抗(脾肾阳虚证)的MHD患者采用中西医结合疗法治疗的效果显著,可有效改善贫血相关指标,且安全性较好.
Objective:To explore the clinical effect of integrated traditional Chinese and western medicine therapy in the treatment of maintenance hemodialysis(MHD)patients with Erythropoietin(EPO)resistance(spleen and kidney Yang deficiency syndrome).Method:A total of 70 MHD patients with EPO resistance(spleen and kidney Yang deficiency syndrome)admitted to Ganzhou Municipal Hospital from February 2022 to February 2025 were prospectively selected and divided into the control group(n=35)and the observation group(n=35)according to the random number table method.The control group was treated with EPO combined with Polysaccharide Iron Complex,while the observation group was treated with integrated traditional Chinese and western medicine therapy(Roxadustat,Polysaccharide Iron Complex and Shengxuening Tablets).The treatment course for both groups was 3 months.The clinical efficacy,anemia indicators,traditional Chinese medicine syndrome scores,inflammatory factors,iron metabolism indicators and the incidence of adverse reactions of the two groups were compared.Result:After treatment,the total effective rate of the observation group was higher than that of the control group(P<0.05).After treatment,the main symptom score,secondary symptom score and total score of the traditional Chinese medicine syndrome score in the observation group were all lower than those in the control group,while the levels of hemoglobin,hematocrit,serum iron,transferrin saturation and ferritin in the observation group were higher than those in the control group,and the levels of nuclear factor-κB and interferon-γ were lower than those in the control group(P<0.05).The comparison of the incidence of adverse reactions between the two groups showed no statistically significant difference(P>0.05).Conclusion:MHD patients with EPO resistance(spleen and kidney Yang deficiency syndrome)have shown remarkable therapeutic effects when treated with integrated traditional Chinese and western medicine therapy.It can effectively improve their anemia-related indicators and has good safety.
