顺铂腹腔热灌注化疗联合静脉化疗治疗晚期上皮性卵巢癌的疗效分析OA
Efficacy of cisplatin-based hyperthermic intraperitoneal chemotherapy combined with intravenous chemotherapy for advanced epithelial ovarian cancer
目的 探讨顺铂腹腔热灌注化疗(HIPEC)联合静脉化疗在晚期上皮性卵巢癌(EOC)治疗中的临床价值.方法 收集2021年1月至2025年12月间中国医科大学附属第一医院妇科病房收治的60例晚期EOC患者的临床资料.根据治疗方案分为研究组(肿瘤细胞减灭术+顺铂HIPEC+紫杉醇联合卡铂化疗,15例)与对照组(肿瘤细胞减灭术+紫杉醇联合卡铂化疗,45例),比较2组患者治疗后客观缓解率(ORR);治疗前后血清癌抗原12-5(CA12-5)、人附睾蛋白4(HE4)、可溶性程序性死亡受体配体1(sPD-L1)、可溶性T细胞免疫球蛋白黏蛋白结构域3(sTIM-3)及T淋巴细胞亚群水平.比较2组患者2年总生存率及治疗相关不良反应发生率.结果 研究组患者ORR显著高于对照组(93.33%vs.82.22%,P<0.05).治疗后2组血清CA12-5、HE4、sPD-L1、sTIM-3水平均较治疗前显著下降(P<0.05),且研究组各项指标下降幅度均显著大于对照组(P<0.05).治疗后2组CD3+T细胞、CD4+T细胞百分比及CD4+T细胞/CD8+T细胞比值均较治疗前升高,CD8+T细胞百分比下降;研究组上述指标的改善程度均显著优于对照组(P<0.05).在生存期方面,研究组2年总生存率为100%,高于对照组(86.67%),但2组间比较差异无统计学意义(P>0.05).在安全性方面,2组不良反应发生率差异无统计学意义(P>0.05).结论 顺铂HIPEC联合静脉化疗可显著提高晚期EOC患者的治疗反应率,有效降低肿瘤负荷;2年总生存率有提高趋势,且未增加严重不良反应风险.其机制可能与下调sPD-L1、sTIM-3等免疫抑制分子、改善T淋巴细胞亚群分布、调节免疫有关.此联合方案安全可行,具有较好的临床应用前景.
Objective To evaluate the clinical value of cisplatin-based hyperthermic intraperitoneal chemotherapy(HIPEC)combined with intravenous chemotherapy in the treatment of advanced epithelial ovarian cancer(EOC),focusing on its effects on serum tumor markers,immune checkpoint molecules,and immunomodulation.Methods A total of 60 patients with advanced EOC who were admitted to the Department of Gynecology,the First Hospital of China Medical University,between January 2021 and December 2025 were enrolled and divided into the study(n=15)and control(n=45)groups according to treatment regimens.The study group received cytoreductive surgery+cisplatin-based HIPEC+paclitaxel combined with carboplatin chemotherapy,whereas the control group received cytoreductive surgery+paclitaxel combined with carboplatin chemotherapy.The objective response rate(ORR)was compared between the two groups.Serum levels of cancer antigen 12-5(CA12-5),human epididymis protein 4(HE4),soluble programmed death-ligand 1(sPD-L1),soluble T-cell immunoglobulin and mucin-domain containing-3(sTIM-3),and T lymphocyte subsets(CD3+,CD4+,and CD8+T cells,as well as the CD4+/CD8+T cell ratio)were measured before and after treatment.In addition,the 2-year overall survival rate and incidence of treat-ment-related adverse reactions were compared to evaluate survival benefits and safety.Results The ORR in the study group was signif-icantly higher than that in the control group[93.33%(14/15)vs.82.22%(37/45),P=0.019].After treatment,serum levels of CA12-5,HE4,sPD-L1,and sTIM-3 decreased significantly in both groups compared with those before treatment(P<0.05),and the decreases in all indicators were significantly greater in the study group than in the control group(P<0.05).After treatment,the percentages of CD3+and CD4+T cells and the CD4+/CD8+T cell ratio increased in both groups,whereas the percentage of CD8+T cells decreased;the improve-ments in these indicators were significantly better in the study group than in the control group(P<0.05).The 2-year overall survival rate was 100%in the study group compared with 86.67%in the control group;however,this difference was not statistically significant(P>0.05).In terms of safety,no statistically significant difference was observed in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Cisplatin-based HIPEC combined with intravenous chemotherapy can significantly improve the treatment response rate in patients with advanced EOC and effectively reduce tumor burden.The combined regimen exhibits a trend toward improved 2-year overall survival without increasing the risk of severe adverse reactions.The mechanism may be related to the downregulation of immuno-suppressive molecules such as sPD-L1 and sTIM-3,improvement of T lymphocyte subset distribution,and immunomodulation.This com-bined regimen is safe and feasible,with promising clinical application prospects.
