银杏达莫联合丁苯酞对缺血性脑卒中后认知功能障碍患者认知及神经功能的影响OA
Effects of ginkgo-damole combined with butylphthalide on cognition and neurological function in patients with post-ischemic stroke cognitive impairment
目的 探讨银杏达莫联合丁苯酞对缺血性脑卒中后认知功能障碍患者认知及神经功能的影响.方法选取2023年3月—2024年10月邢台市中心医院收治的146例缺血性脑卒中后认知功能障碍患者,按照随机数字表法分为试验组(银杏达莫联合丁苯酞治疗)、对照组(丁苯酞治疗),各73例.对比两组疗效状况、认知状况、神经功能、血液流变学[血浆黏度(PV)、全血黏度(BV)、红细胞沉降率(ESR)、红细胞压积(HCT)]、炎症因子[白细胞介素-1β(IL-1β)、白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)]、神经递质水平[5-羟色胺(5-HT)、去甲肾上腺素(NE)、多巴胺(DA)]的变化.结果 试验组总有效率高于对照组(P<0.05).试验组治疗后蒙特利尔认知评估量表(MoCA)、简易智能精神状态检查量表(MMSE)评分均高于对照组(P<0.05),两组治疗后MoCA、MMSE评分均高于治疗前(P<0.05).试验组治疗后美国国立卫生研究院卒中量表(NIHSS)、神经元特异性烯醇化酶(NSE)、中枢神经特异性蛋白(S100β)水平均低于对照组(P<0.05),两组治疗后NIHSS、NSE、S100β水平均低于治疗前(P<0.05).试验组治疗后PV、BV、ESR、HCT均低于对照组(P<0.05),两组治疗后PV、BV、ESR、HCT均低于治疗前(P<0.05).试验组治疗后IL-1β、IL-6、TNF-α均低于对照组,5-HT、NE、DA均高于对照组(P<0.05);两组治疗后IL-1β、IL-6、TNF-α均低于治疗前,5-HT、NE、DA均高于治疗前(P<0.05).结论 银杏达莫联合丁苯酞治疗可进一步增强临床疗效,促进患者认知能力及神经功能恢复,同时对血液流变学相关指标、炎症因子和神经递质水平起到积极调控作用.
Objective To investigate the effects of ginkgo-damole combined with butylphthalide on cognition and neurological function in patients with post-ischemic stroke cognitive impairment.Methods A total of 146 patients with post-ischemic stroke cognitive impairment admitted to Xingtai Central Hospital from March 2023 to October 2024 were selected and randomly divided into an experimental group(73 cases,treated with ginkgo-damole combined with butylphthalide)and a control group(73 cases,treated with butylphthalide alone)using a random number table method.The therapeutic efficacy,cognitive status,neurological function,hemorheological parameters[plasma viscosity(PV),whole blood viscosity(BV),erythrocyte sedimentation rate(ESR),hematocrit(HCT)],inflammatory factors[interleukin-1β(IL-1β),interleukin-6(IL-6),tumor necrosis factor-α(TNF-α)],and neurotransmitter levels[5-hydroxytryptamine(5-HT),norepinephrine(NE),dopamine(DA)]were compared between the two groups.Results The overall effective rate in the experimental group was higher than that in the control group(P<0.05).After treatment,the Montreal Cognitive Assessment(MoCA)and Mini-Mental State Examination(MMSE)scores in the experimental group were higher than those in the control group(P<0.05),and both scores were significantly improved compared with baseline values in both groups(P<0.05).After treatment,the National Institutes of Health Stroke Scale(NIHSS)scores,and levels of NSE and S100β in the experimental group were lower than those in the control group(P<0.05),and all were significantly reduced compared with baseline values in both groups(P<0.05).After treatment,PV,BV,ESR,and HCT in the experimental group were lower than those in the control group(P<0.05),and all were significantly decreased compared with baseline values in both groups(P<0.05).After treatment,IL-1β,IL-6,and TNF-α levels in the experimental group were lower than those in the control group(P<0.05),and all were significantly reduced compared with baseline values in both groups(P<0.05).After treatment,5-HT,NE,and DA levels in the experimental group were higher than those in the control group(P<0.05),and all were significantly increased compared with baseline values in both groups(P<0.05).Conclusion Ginkgo-damole combined with butylphthalide can further enhance clinical therapeutic efficacy,promote the recovery of cognitive ability and neurological function,and exert positive regulatory effects on hemorheological parameters,inflammatory factors,and neurotransmitter levels.
