温阳通督针刺联合重复经颅磁刺激对脑卒中后偏瘫患者肌力及运动功能的影响OA
Effect of Wenyang Tongdu Acupuncture combined with repetitive transcranial magnetic stimulation on muscle strength and motor function in patients with post-stroke hemiplegia
目的 探究温阳通督针刺联合重复经颅磁刺激(rTMS)治疗脑卒中后偏瘫患者的疗效.方法 选取2023年10月—2026年1月于皖南医学院第二附属医院接受诊治的80例脑卒中后偏瘫患者,采用随机数字表法将其分为对照组和研究组,各40例.两组患者接受常规治疗,对照组患者接受rTMS治疗,研究组在此基础上联合温阳通督针刺治疗.评估两组患者治疗4周后临床疗效,比较治疗前、治疗4周后两组患者肌力、运动功能[肢体运动评分量表(FMA)]、平衡功能[Berg平衡量表(BBS)]、日常生活活动能力[改良barthel指数(MBI)]的差异,统计两组患者治疗期间不良反应发生情况.结果 治疗4周后,研究组治疗总有效率(92.5%)高于对照组(72.5%)(P<0.05).研究组治疗后肌力评分、FMA各项评分(上肢、下肢、肌力、协调、感觉)、BBS评分、MBI评分均高于对照组(P<0.05),两组治疗后肌力评分、FMA各项评分、BBS评分、MBI评分均高于治疗前(P<0.05).两组不良反应总发生率比较,差异无统计学意义(P>0.05).结论 温阳通督针刺联合rTMS能增强脑卒中后偏瘫患者肌力,促进运动功能和平衡功能恢复,提高日常生活活动能力,临床疗效确切且安全性良好.
Objective To explore the efficacy of Wenyang Tongdu Acupuncture combined with repetitive transcranial magnetic stimulation(rTMS)in the treatment of patients with post-stroke hemiplegia.Methods A total of 80 patients with post-stroke hemiplegia who were diagnosed and treated at our hospital from October 2023 to January 2026 were selected and divided into the control group and the study group by the random number table method,with 40 cases in each group.Both groups received routine treatment,and the control group received rTMS treatment.Additionally,the study group was also treated with Wenyang Tongdu Acupuncture.The clinical efficacy in the two groups after 4 weeks of treatment was evaluated.The differences in muscle strength,motor function[Fugl-Meyer Assessment(FMA)scores],balance function[Berg Balance Scale(BBS)scores]and activities of daily living[Modified Barthel Index(MBI)scores]were compared between the two groups of patients before treatment and after 4 weeks of treatment.The adverse reactions during treatment in the two groups were statistically analyzed.Results After 4 weeks of treatment,the overall response rate in the study group(92.5%)was higher than that in the control group(72.5%)(P<0.05).After treatment,the muscle strength scores,all subscale scores of the FMA(including upper extremity,lower extremity,muscle strength,coordination,sensation),BBS scores,and MBI scores in the study group were higher than those in the control group(P<0.05).In both groups,the muscle strength scores,all subscale scores of the FMA,BBS scores,and MBI scores after treatment were higher than those before treatment(P<0.05).There was no statistically significant difference in the overall incidence of adverse reactions between the two groups(P>0.05).Conclusion Wenyang Tongdu Acupuncture combined with rTMS can enhance muscle strength,promote the recovery of motor function and balance function,and increase the activities of daily living in patients with post-stroke hemiplegia,with definite clinical efficacy and good safety.
