Updates to the Rome Ⅴ criteria for pediatric disorders of gut-brain interaction
罗马Ⅴ儿科标准对儿童肠-脑互动异常(disorders of gut-brain interaction,DGBI)诊疗体系进行了系统性重构.该标准打破罗马N按年龄分层的分类模式,建立全年龄段统一的诊断框架,依据解剖部位和症状模式划分为上消化道与下消化道和胆道疾病两大类,新增13项疾病诊断,填补儿科命名空白.标准首次系统构建以生物-心理-社会模型为核心的病理生理体系,细化诊断鉴别逻辑,并采纳欧洲儿科胃肠病学、肝病学和营养学会及北美儿科胃肠病学、肝病学和营养学会(ESPGHAN/NASPGHAN)指南规范非药物、饮食及药物分层治疗.我国儿科临床应在借鉴国际标准基础上,结合婴幼儿功能性消化不良综合征本土共识及中西医诊疗经验,推动多学科协作,提升儿童DGBI规范化诊疗水平.
The Rome Ⅴ Pediatric Criteria have systematically reconstructed the diagnosis and treatment system for Disorders of Gut-Brain Interaction(DGBI)in children.Breaking the age-stratified classification model adopted by RomeⅣ,the criteria establish a unified diagnostic framework applicable to all age groups.They categorize conditions into two major groups:upper gastrointestinal tract disorders and lower gastrointestinal and biliary tract disorders based on anatomical locations and symptom patterns,adding 13 new disease diagnoses to fill the gaps in pediatric nomenclature.For the first time,the criteria systematically construct a pathophysiological system centered on the biopsychosocial model,refine the logic for differential diagnosis,and adopt the guidelines from ESPGHAN/NASPGHAN to standardize tiered non-pharmacological,dietary and pharmacological therapies.Pediatric clinical practice in China should draw on these international criteria while integrating domestic consensus on infant functional dyspepsia syndrome as well as Chinese and Western medicine diagnosis and treatment experience,promoting multidisciplinary collaboration,and elevate the standardized diagnosis and treatment of pediatric DGBI.
