城市化背景下腹泻型肠易激综合征与代谢综合征的共病机制OA
The comorbidity mechanism of diarrhea predominant irritable bowel syndrome and metabolic syndrome in the context of urbanization
城市化社会背景下,随着现代饮食结构改变和生活压力的增加,腹泻型肠易激综合征(IBS-D)发病率逐年上升,极大影响了人们的生活质量.与此同时,肥胖、糖尿病、代谢功能障碍相关脂肪性肝病等代谢性疾病亦日趋增多,代谢综合征(MetS)相关的多系统损害成为人们关注的研究热点.目前,针对两种疾病的独立发病机制已形成共识,而对共病的病理机制以及生理交互作用仍不明确.基于此,本文以脑-肠轴失衡为切入点,对两者共病研究进行了系统回顾与梳理,并从"脾主运化"角度探讨了"异病同治"的中医理论内涵,以期突破IBS-D、MetS共病的治疗瓶颈,优化临床管理策略,为中西医有机结合提供思路与借鉴.
In an urbanized society,changes in dietary patterns and increased life stress have led to a steady annual rise in the incidence of diarrhea predominant irritable bowel syndrome(IBS-D),substantially impairing patients' quality of life.Concurrently,metabolic disorders such as obesity,diabetes,and metabolic dysfunction-associted steatotic liver disease are also increasing,and multisystem damage associated with metabolic syndrome(MetS)has drawn considerable research attention.Although the independent pathogenesis of each condition is well recognized,the pathological mechanisms and physiological interactions underlying their comorbidity remain unclear.Accordingly,this review uses the brain-gut axis imbalance as a conceptual framework to systematically synthesize and summarize the current evidence on their co-occurrence.In addition,we explore the traditional Chinese medicine theory of"treating different diseases with the same approach"from the perspective that the spleen governs transportation and transformation.This integrative perspective aims to overcome current therapeutic bottleneck in managing IBS-D and MetS comorbidity,optimize clinical management strategies,and offer insights and references for the integration of Chinese and Western medicine.
