目的:本研究旨在应用剪切波弹性成像对单侧腰椎间盘突出症患者腰部竖脊肌、多裂肌、腰大肌、腰方肌进行定量评估和对比分析,探索腰椎间盘突出症诊断中剪切波弹性成像技术的临床应用价值.方法:招募单侧腰椎间盘突出症患者30例,运用剪切波弹性成像技术,在俯卧位下对受试者双侧腰部竖脊肌、多裂肌、腰大肌、腰方肌进行探查,获取双侧竖脊肌、多裂肌、腰大肌、腰方肌的剪切波杨氏模量,分析单侧腰椎间盘突出患者患侧肌肉剪切波杨氏模量与腰痛程度[视觉模拟量表(VAS)评分]和功能障碍[Oswestry功能障碍指数(ODI)评分]的相关性.结果:在单侧腰椎间盘突出患者中,患侧多裂肌、竖脊肌杨氏模量显著降低(P<0.01),且与 VAS和 ODI评分呈负相关(P<0.01);患侧腰方肌杨氏模量显著增高(P<0.01),且与 VAS和 ODI评分呈正相关(P<0.01);而腰大肌杨氏模量两侧无统计学差异,且与临床评分不相关.结论:剪切波弹性成像技术可以定量、实时、有效地评估竖脊肌、多裂肌、腰大肌、腰方肌的硬度,为腰椎间盘突出的临床诊断提供客观依据.

Objective:To quantitatively evaluate and comparatively analyze the erector spinae,multifidus,psoas major,and quadratus lumborum muscles in the lumbar region of patients with unilateral lumbar disc herniation u-sing shear wave elastography,and to explore the clinical application value of shear wave elastography in the diagno-sis of lumbar disc herniation.Methods:All 30 patients with unilateral lumbar disc herniation meeting the criteria were recruited at Beijing Bo'ai Hospital.Shear wave elastography was used to examine the erector spinae,multifi-dus,psoas major,and quadratus lumborum muscles on both sides of the waist of the subjects in the prone position,and the shear wave Young's modulus of the erector spinae,multifidus,psoas major,and quadratus lumborum mus-cles on both sides were obtained.The correlation between the shear wave Young's modulus of the affected side muscles in patients with unilateral lumbar disc herniation and the degree of low back pain[(visual analogue scale,VAS)score]and[(Oswestry disability index,ODI)score]were analyzed.Results:In patients with unilateral lum-bar disc herniation,the Young's modulus of the multifidus and erector spinae muscles on the affected side was sig-nificantly decreased(P<0.01),and was negatively correlated with VAS and ODI scores(P<0.01);the Young's modulus of the quadratus lumborum muscle on the affected side was significantly increased(P<0.01),and was positively correlated with VAS and ODI scores(P<0.01);while there was no significant difference in the Young's modulus of the psoas major muscle between the two sides,and it was not correlated with clinical scores.Conclu-sion:The shear wave Young's modulus of the erector spinae,psoas major and quadratus lumborum muscles in pa-tients with lumbar intervertebral disc protrusion is greater than that of normal individuals.The shear wave elastog-raphy technique can effectively and objectively evaluate the hardness of the erector spinae,psoas major and quadra-tus lumborum muscles,providing an effective basis for the clinical diagnosis of lumbar disc protrusion.
