Beckman口部运动训练联合喉部振动治疗对脑瘫患儿吞咽功能改善的临床效果OA
Efficacy of Beckman oral motor training combined with laryngeal vibration therapy for dysphagia in children with cere-bral palsy
目的:探讨Beckman口部运动训练联合100Hz喉部振动治疗对脑性瘫痪(CP)伴口咽期吞咽障碍患儿的临床疗效.方法:选择CP伴吞咽障碍患儿68例,采用分层区组随机法,将其随机分为2组,每组34例.对照组采用常规吞咽障碍康复治疗,观察组在对照组基础上给予喉部振动治疗.治疗周期为4周.分别于治疗前和治疗后采用标准吞咽功能 评 价 量 表(SSA)、饮食能力分级系统(EDACS)、渗透-误吸评价量表(PAS)和表面肌电信号(sEMG)评价患儿吞咽功能.结果:治疗后,2组SSA评分均较治疗前显著降低(P<0.01),且观察组显著优于对照组(P<0.01);2组EDACS明显降低(P<0.01),且观察组显著优于对照组(P<0.01);观察组PAS评分明显降低(P<0.01),且明显低于同期对照组(P<0.01);2组sEMG振幅及峰值明显升高(P<0.01),且观察组振幅及峰值均高于对照组(P<0.01).结论:Beckman口部运动训练联合喉部振动治疗能显著提升CP患儿的口咽期吞咽能力,减少误吸风险,其效果优于单一的口部运动训练,具有较好的临床推广价值.
Objective:To observe the synergistic effect of Beckman oral motor training combined with 100 Hz la-ryngeal vibration therapy on oropharyngeal dysphagia in children with cerebral palsy(CP).Methods:A total of 68 children with CP and dysphagia admitted to the Children's Hospital Affiliated to Soochow University from June 2023 to December 2024 were selected.Using the stratified block randomization method(stratified by GMFCS classi-fication,block length=4),they were divided into an observation group and a control group.The control group re-ceived conventional rehabilitation treatment for dysphagia.The observation group was additionally given laryngeal vibration therapy on this basis.The swallowing function of the children was evaluated using the Eating and Drinking Ability Classification System(EDACS),Standardized Swallowing Assessment(SSA),Penetration-Aspiration Scale(PAS),and surface electromyography(sEMG)before treatment and after 4 weeks of treatment.Results:After 4 weeks of treatment,SSA scores and EDACS levels in both groups significantly decreased compared with those be-fore treatment(P<0.01),and these scores were significantly lower in the observation group than in the control group(P<0.01).The PAS score of the observation group decreased significantly(P<0.01).After treatment,the observation group was significantly better than the control group in terms of VFSS and PAS(P<0.01).The sEMG amplitude and peak value significantly increased in both groups,with statistically significant differences(P<0.01),and those in the observation group were significantly better than in the control group(P<0.01).Conclu-sion:Beckman oral motor training combined with laryngeal vibration therapy can significantly improve dysphagia in children with CP,and its efficacy is better than oral motor training alone.This combined therapy is worthy of pro-motion and application in children with CP complicated with dysphagia.
