目的:探讨分级运动想象联合核心肌群训练对脑卒中患者下肢运动和平衡功能的临床疗效.方法:选取70例脑卒中偏瘫患者,按疾病亚型与病程分层后随机分为对照组与观察组,各35例.2组均行常规康复训练,对照组加用躯干核心肌群训练,观察组在此基础上联合分级运动想象训练,均干预4周.采用 Fugl-Meyer下肢运动功能评定量表(FMA-LE)、功能步行分级(FAC)、Berg平衡量表(BBS)、改良 Barthel指数(MBI)、下肢 Brunnstorm分期、Tinetti步态分析量表(TGA)评估疗效.结果:治疗4周后,3组FAC、下肢 Brunnstorm 分期均较治疗前显著改善(P<0.01),且观察组更优(P<0.05,0.01);2组 FMA-LE、BBS、TGA、MBI评分均较治疗前显著升高(P<0.01),观察组上述指标及改善差值(除 MBI外)均显著高于对照组(P<0.05,0.01).结论:分级运动想象联合核心肌群训练可更有效改善脑卒中患者下肢运动与平衡功能,提高日常生活活动能力,优于单纯常规康复训练联合核心肌群训练.

Objective:To investigate the clinical efficacy of graded motor imagery combined with core muscle train-ing on lower limb movement and balance function in stroke patients.Methods:A total of 70 patients with hemiple-gia due to stroke were selected and randomly divided into a control group and an observation group(n=35 each)based on disease subtype and disease duration.Both groups received standard rehabilitation training.The control group additionally underwent trunk core muscle training,while the observation group received combined graded mo-tor imagery training.The intervention lasted for 4 weeks.Therapeutic efficacy was assessed using the Fugl-Meyer lower extremity motor function scale(FMA-LE),functional ambulation classification(FAC),Berg balance scale(BBS),modified Barthel index(MBI),lower extremity Brunnstrom staging,and Tinetti gait analysis scale(TGA).Results:After 4 weeks of treatment,both groups showed significant improvements in FAC scores and low-er limb Brunnstrom staging compared to baseline(P<0.01),with the observation group demonstrating superior outcomes(P<0.05,0.01).Additionally,all FMA-LE,BBS,TGA,and MBI scores increased significantly in both groups(P<0.01),with the observation group exhibiting significantly higher values and greater improvement differences(except for MBI)than the control group(P<0.05,0.01).Conclusion:Graded motor imagery com-bined with core muscle training is more effective than conventional rehabilitation training alone in improving lower limb motor and balance functions as well as enhancing daily living activities in stroke patients.
