Research Progress on Molecular Mechanisms and Precision Diagnosis and Treatment of Spinal Organotropic Metastasis in Lung Adenocarcinoma

肺腺癌(lung adenocarcinoma,LUAD)是非小细胞肺癌(non-small cell lung cancer,NSCLC)中最常见的亚型,脊柱转移发生率高且预后较差.LUAD的脊柱转移并非单纯的骨转移,而是由脊柱解剖特性、微环境特异性及分子调控机制共同介导的定向转移过程,但现有研究多聚焦于泛骨转移,针对脊柱定向转移的专属机制与精准诊疗体系尚未形成.因此,本文从LUAD脊柱定向转移潜能、微环境调控、转移途径3个关键方面,系统阐述其分子机制,并结合脊柱转移的临床特点与当前研究不足,为LUAD脊柱转移的基础研究和临床精准防治提供新思路.

Lung adenocarcinoma(LUAD)is the most common subtype of non-small cell lung cancer(NSCLC),with a high incidence of spinal metastasis and poor prognosis.Spinal metastasis of LUAD is not merely a simple bone metas-tasis but a directed metastasis process mediated by the anatomical characteristics of the spine,microenvironment specificity,and molecular regulatory mechanisms.However,current research mainly focuses on general bone metastasis,and a dedicated mechanism and precise diagnosis and treatment system for spinal-directed metastasis have not yet been established.Therefore,this article systematically elaborates on the molecular mechanisms of LUAD spinal-directed metastasis from three key aspects:metastatic potential,microenvironment regulation,and metastasis pathways.Combined with the clinical characteristics of spi-nal metastasis and the current research deficiencies,it provides new ideas for basic research and clinical precise prevention and treatment of LUAD spinal metastasis.
