目的:探讨暖宫止痛贴(NGZT)对寒凝血瘀型原发性痛经(PD)大鼠的作用机制.方法:采用冰水浴联合盐酸肾上腺素建立大鼠寒凝血瘀型,苯甲酸雌二醇联合缩宫素制备寒凝血瘀型PD大鼠模型.以布洛芬为阳性对照组,观察大鼠扭体次数和潜伏时间;酶联免疫吸附测定(ELISA)法检测血清前列腺素E2(PGE2)、前列腺素F2α(PGF2α)含量;苏木精-伊红(HE)染色观察子宫形态;免疫荧光检测 MVD;蛋白质印迹法(Western Blot)和逆转录聚合酶链反应(RT-PCR)法检测子宫组织磷脂酰肌醇3-激酶/蛋白激酶B(PI3K/Akt)/环氧化酶-2(COX-2)/血管内皮生长因子(VEGF)通路蛋白及信使核糖核酸(mRNA)表达.结果:NGZT组大鼠扭体次数显著减少、潜伏期延长,差异具有统计学意义(P<0.01),血清中PGE2 水平升高,PGF2α水平及PGF2α/PGE2 比值降低,差异具有统计学意义(P<0.01),子宫组织水肿及炎症浸润减轻,COX-2、VEGF蛋白与基因表达及 MVD下调,差异具有统计学意义(P<0.01),暖宫止痛贴(NGZT)效果与布洛芬相当,但对PI3K/AKT通路无显著影响,差异无统计学意义(P>0.05).结论:NGZT能有效改善大鼠寒凝血瘀型PD,其作用机制可能通过抑制COX-2/PGE2 炎症通路与 VEGF/微血管密度(MVD)血管新生通路,但与PI3K/AKT信号通路无显著联系.

Objective:To investigate the mechanism of action of Nuangong Zhitong plaster(NGZT)on rats with prima-ry dysmenorrhea(PD)of cold-congealed blood stasis type.Methods:The rat cold-congealed blood stasis syndrome was established by ice-water bath combined with epinephrine hydrochloride,estradiol benzoate combined with oxytocin to prepare a rat cold-congealed blood stasis type PD.Ibuprofen was used as the positive control,the number of writhing movements and latency time in rats were observed;serum levels of Prostaglandin E2(PGE2)and Prostaglandin F2α(PGF2α)were detected by Enzyme-Linked Immunosorbent Assay(ELISA);uterine morphology was observed by He-matoxylin-Eosin(HE)staining;microvessel density(MVD)was detected by immunofluorescence;protein and mRNA expression of PI3K/AKT/COX-2/VEGF pathway in uterine tissue was detected by Western blot and RT-PCR.Re-sults:NGZT significantly reduced the number of writhing movements and prolonged the latency time of writhing move-ments(P<0.01),elevated the levels of PGE2,lowered the levels of PGF2α inserum,and decreased PGF2α/PGE2 ratio,reduced the degree of edema and inflammatory infiltration in uterus,and downregulated protein and gene expression of COX-2,VEGF and MVD(P<0.01).The effect was comparable to that of ibuprofen,but no significant effect was ob-served on the PI3K/AKT pathway(P>0.05).Conclusion:The mechanism by which NGZT effectively improved cold-congealed blood stasis type PD in rat maybe inhibiting the COX-2/PGE2 inflammation pathway and the VEGF/MVD angiogenesis pathway,independently of the PI3K/AKT signalling pathway.
