目的 观察宫粘2 号方对宫腔粘连大鼠子宫内膜及腺体损伤的影响,并基于磷脂酰肌醇3-激酶(PI3K)/蛋白激酶 B(Akt)信号通路探讨其作用机制.方法 取60 只 SPF 级雌性 SD大鼠,随机选择10 只作为假手术组,其余50 只采取机械刮宫+脂多糖感染的方法制备宫腔粘连模型.将造模成功大鼠随机分为模型组、西药组、宫粘2 号方低剂量组、宫粘2 号方中剂量组、宫粘2号方高剂量组,每组9 只.宫粘2 号方低、中、高剂量组分别灌胃浓度为 0.675 g/mL、1.35 g/mL、2.7g/mL 的宫粘2 号方汤剂,西药组灌胃浓度为0.018 g/mL 的戊酸雌二醇片溶液,假手术组和模型组灌胃生理盐水,灌胃容积均为10 mL/(kg·d),每天 1 次,连续灌胃 21 d.解剖大鼠并肉眼观察子宫外观,HE 染色观察大鼠子宫组织病理形态,计算子宫内膜厚度及腺体数,Western blot 法检测子宫组织中 CXC 基序趋化因子配体5(CXCL5)、PI3K、Akt、基质金属蛋白酶-9(MMP-9)蛋白表达情况.结果 与假手术组比较,模型组子宫外观可见多处缩窄,局部积水膨大,色泽稍暗,质地僵硬,弹性差;HE 染色见子宫内膜粘连,有较多炎症细胞浸润,组织纤维化增生,且子宫内膜厚度、腺 体数均明显减少(P均<0.05);子宫组织中CXCL5、MMP-9蛋白相对表达量均明显降低(P均<0.05),PI3K、Akt 蛋白相对表达量均明显升高(P 均<0.05).与模型组比较,各药物组大鼠子宫外观色泽较红润,质地柔软,弹性较好;宫腔较通畅,炎性细胞浸润及组织纤维化减少,子宫内膜厚度、腺体数均明显增多(P 均<0.05);子宫组织中 CXCL5、MMP-9 蛋白相对表达量均明显升高(P 均<0.05),PI3K、Akt 蛋白相对表达量均明显降低(P 均<0.05).与西药组和宫粘2 号方低剂量组比较,宫粘2 号方中、高剂量组大鼠子宫内膜厚度、腺体数及子宫组织中 CXCL5、MMP-9、PI3K、Akt 蛋白相对表达量均改善更明显(P 均<0.05).结论 宫粘2 号方能减轻宫腔粘连大鼠子宫内膜及腺体损伤,其机制可能与上调 CXCL5 表达,抑制 PI3K/Akt 信号通路,从而上调 MMP-9 表达有关.

Objective It is to observe the effect of Gongzhan No.2 formula on endometrial and glandular injury in rats with uterine adhesions,and to explore its mechanism of action based on the phosphoinositide 3-kinase(PI3K)/protein ki-nase B(Akt)signaling pathway.Methods Sixty SPF grade female SD rats were taken,10 of which were randomly selected as sham operation group,and the 50 remaining rats were prepared with mechanical curettage and lipopolysaccharide infec-tion to establish models of uterine adhesions.The successfully modeled rats were randomly divided into model group,west-ern medicine group,the low-dose,medium-dose and high-dose groups of Gongzhan No.2 formula,with 9 rats in each group.The low-dose,medium-dose and high-dose groups of Gongzhan No.2 formula were administered decoctions of Gongzhan No.2 formula via gavage at concentrations of 0.675 g/mL,1.35 g/mL,and 2.7 g/mL,respectively,the western medicine group was administered a solution of estradiol valerate tablets at a concentration of 0.018 g/mL via gavage,while the sham opera-tion group and model group were administered normal saline via gavage.The administration volume was 10 mL/(kg·d)in each group,once daily,continuously treated for 21 days.The rats were dissected,and the appearance of their uterus was examined with naked eyes,the histopathological morphology of uterine tissue was observed by HE staining,the thickness of endometrium and the number of glands were measured,the protein expressions of CXC chemokine ligand 5(CXCL5),PI3K,Akt,and matrix metalloproteinase-9(MMP-9)in uterine tissue were detected by Western blot.Results Compared with the sham operation group,the uterus of rats in the model group exhibited multiple areas of visible narrowing,localized fluid accumulation and distension,a slightly darker color,a stiff texture,and poor elasticity;HE staining revealed that there were endometrial adhesions,extensive inflammatory cell infiltration,and fibrotic hyperplasia in uterus;furthermore,the endometrial thickness and the number of glands were significantly reduced(all P<0.05),the relative protein expres-sions of CXCL5 and MMP-9 in the uterine tissue were both significantly decreased(both P<0.05),while the relative pro-tein expressions of PI3K and Akt were both significantly increased(both P<0.05).Compared with the control group,the uterus of rats in the drug-treated groups was ruddy in appearance,soft in texture with good elasticity;the uterine cavity was more patent,with reduced inflammatory cell infiltration and tissue fibrosis,and the endometrial thickness and the number of glands were significantly increased(both P<0.05);the relative protein expressions of CXCL5 and MMP-9 in uterine tis-sue were significantly increased,while the relative protein expressions of PI3K and Akt were significantly reduced(all P<0.05).Compared with the western medicine group and low-dose group of Gongzhan No.2 formula,the improvements of en-dometrial thickness,gland count,and the relative protein expressions of CXCL5,MMP-9,PI3K and Akt in uterine tissue were more significant in the medium-dose and high-dose groups of Gongzhan No.2 formula(all P<0.05).Conclusion Gongzhan No.2 formula can alleviate endometrium and glands injury in rats with uterine adhesions,its mechanism of action may be related to up-regulating CXCL5 expression and inhibiting the PI3K/Akt signaling pathway,thereby up-regulating MMP-9 expression.
