目的 观察苏木乙酸乙酯提取物对急性心肌梗死大鼠冠脉血管新生及磷脂酰肌醇3-激酶(PI3K)/蛋白激酶 B(Akt)信号通路的影响,探讨其对心肌损伤影响的机制.方法 将 80 只雄性 SD 大鼠随机分为空白组、假手术组、模型组、苏木组,每组20 只.空白组正常饲养,假手术组开胸穿线后不结扎左冠状动脉前降支,模型组和苏木组结扎左冠状动脉前降支建立急性心肌梗死模型.术后清醒后,苏木组给予苏木乙酸乙酯提取物50 mg/kg 灌胃,其他组灌胃等体积蒸馏水,均1 次/d,连续灌胃14 d.术后第1,7,14 天通过 TTC 染色测定各手术组大鼠心肌梗死面积占比;末次灌胃结束后,HE 染色观察心肌组织病理形态及梗死边缘区心肌微血管密度,Matrigel 结合流式细胞术计算 Matrigel 胶栓血管新生指数,RT-qPCR 法检测心肌组织中 PI3K、Akt、VEGF mRNA 表达情况,Western blot 法检测心肌组织中 p-PI3K、PI3K、p-Akt、Akt、血管内皮生长因子(VEGF)蛋白表达情况.结果 术后第1,7,14 天,模型组和苏木组大鼠心肌梗死面积占比均明显高于同期假手术组(P 均<0.05);术后第7,14 天,苏木组大鼠心肌梗死面积占比均明显低于同期模型组(P 均<0.05).与假手术组比较,模型组大鼠心肌组织可见大面积坏死灶和心肌纤维断裂;梗死边缘区心肌微血管密度及Matrigel 胶栓血管新生指数均明显升高(P 均<0.05);心肌组织中PI3K、Akt、VEGF mRNA 相对表达量和 p-PI3K/PI3K、p-Akt/Akt 及 VEGF 蛋白相对表达量均明显降低(P 均<0.05).与模型组比较,苏木组大鼠心肌坏死面积明显缩小,微血管数量增多;梗死边缘区心肌微血管密度、Matrigel胶栓血管新生指数均明显升高(P均<0.05);心肌组织中PI3 K、Akt、VEGF mRNA 相对表达量和 p-PI3K/PI3K、p-Akt/Akt 及 VEGF 蛋白相对表达量均明显升高(P 均<0.05).结论 苏木乙酸乙酯提取物可减轻急性心肌梗死大鼠心肌损伤,增加梗死边缘区心肌微血管密度并增强 Matrigel 胶栓血管新生反应,其作用机制可能与激活 PI3K/Akt 信号和上调 VEGF表达相关.

Objective It is to observe the effects of sappanwood ethyl acetate extract on coronary angiogenesis and PI3K/Akt signal pathway in rats with acute myocardial infarction,and to explore its mechanism of action in alleviating myo-cardial injury.Methods Eighty male SD rats were randomly divided into blank group,sham operation group,model group and sappanwood group,with 20 rats in each group.The rats of the control group were normally fed,the rats of the sham op-eration group underwent thoracotomy and threading without ligation of left anterior descending coronary artery,the rats of the model group and sappanwood group underwent ligation of left anterior descending coronary artery to establish acute myo-cardial infarction models.After recovery from anesthesia,the sappanwood group was administrated 50 mg/kg of sappanwood ethyl acetate extract via gavage,while the other groups received an equal volume of distilled water via gavage,all adminis-tered once daily,continuously treated for 14 days.On the 1st,7th and 14th days after surgery,the percentage of myocardi-al infarction area in rats of each surgical group was detected by TTC staining;after the final gastric administration,the pathological morphology of myocardial tissue and the microvascular density in the infarct margin were observed by HE stai-ning,the Matrigel plug angiogenesis index was calculated by flow cytometry,the mRNA expressions of PI3K,Akt,and VEGF in myocardial tissue were detected by RT-qPCR,and the protein expressions of p-PI3K,PI3K,p-Akt,Akt,and vascular endothelial growth factor(VEGF)in myocardial tissue were detected by Western blot.Results On the 1st,7th and 14th days after surgery,the percentages of myocardial infarction area in the model group and sappanwood group were significantly higher than those in the sham operation group at the same period(all P<0.05);on the 7th and 14th days af-ter surgery,the percentages of myocardial infarction area in the sappanwood group were significantly lower than those in the model group at the same period(all P<0.05).Compared with the sham operation group,large areas of necrosis and rup-tured myocardial fibers in myocardial tissue were found in rats of the model group;the microvascular density in infarct mar-gin and the Matrigel plug angiogenesis index were significantly increased(all P<0.05);the relative mRNA expressions of PI3K,Akt and VEGF,as well as the relative protein expressions of p-PI3K/PI3K,p-Akt/Akt and VEGF in myocardial tis-sue were all significantly reduced(all P<0.05).Compared with model group,the area of myocardial necrosis was obvi-ously decreased with increased number of microvessels in rats of the sappanwood group;the microvascular density in infarct margin and the Matrigel plug angiogenesis index were significantly increased(all P<0.05);the relative mRNA expres-sions of PI3K,Akt and VEGF,as well as the relative protein expressions of p-PI3K/PI3K,p-Akt/Akt and VEGF in myo-cardial tissue were all significantly increased(all P<0.05).Conclusion Sappanwood ethyl acetate extract can alleviate myocardial injury,increase microvessel density in infarct margin,and enhance the Matrigel plug angiogenic response in rats with acute myocardial infarction.These effects may be associated with the activation of the PI3K/Akt signaling pathway and the upregulation of VEGF expression.
