免疫治疗背景下小细胞肺癌耐药机制及应对策略OA
Mechanisms of Resistance to Immunotherapy and Therapeutic Strategies in Small Cell Lung Cancer
小细胞肺癌(small cell lung cancer,SCLC)是一种高度侵袭性肿瘤,虽对初始放化疗敏感,但易复发且预后极差.免疫检查点抑制剂的引入改善了广泛期 SCLC 的治疗格局,然而其总体获益有限,耐药几乎不可避免.现有证据表明,SCLC 的免疫耐药并非源于单一通路失活,而是由肿瘤细胞内在免疫惰性、免疫抑制性肿瘤微环境及宿主系统性免疫功能低反应等多层机制共同塑造.肿瘤分子亚型差异、抗原呈递缺陷、表观遗传抑制及谱系可塑性决定了免疫敏感性的基础;同时抑制性髓系细胞、成纤维细胞及异常细胞因子网络进一步削弱效应免疫反应.在此背景下,单一免疫检查点阻断难以实现持久控制.近年来,围绕重塑肿瘤免疫原性、逆转免疫抑制微环境及引导效应免疫的新型联合策略不断发展,尤其是 DNA 损伤通路抑制剂、代谢调控手段以及 Delta 样配体3 靶向抗体-药物偶联物和 T 细胞衔接器的出现,为突破免疫耐药提供了新的方向.系统整合耐药机制与机制驱动的联合策略,有助于推动SCLC 免疫治疗向精准化与可持续获益迈进.
Small cell lung cancer(SCLC)is a highly aggressive malignancy that is initially sensitive to chemotherapy and radiotherapy but is characterized by early relapse and poor prognosis.The introduction of immune checkpoint inhibitors has improved the treatment landscape for extensive-stage SCLC;however,the overall clinical benefit remains limited,and resist-ance to immunotherapy is almost inevitable.Accumulating evidence indicates that immunotherapy resistance in SCLC does not arise from the inactivation of a single immune path-way,but rather from the interplay of multiple mechanisms,in-cluding tumor-intrinsic immune inertia,an immunosuppressive tumor microenvironment,and impaired host systemic immune responsiveness.Molecular subtype heterogeneity,defective an-tigen presentation,epigenetic repression,and lineage plasticity establish the foundation of immune sensitivity,while suppres-sive myeloid cells,cancer-associated fibroblasts,and aberrant cytokine networks further attenuate effective antitumor immune responses.In this context,blockade of a single immune checkpoint is insufficient to achieve durable disease control.In re-cent years,mechanism-based combination strategies aimed at restoring tumor immunogenicity,remodeling the immunosup-pressive microenvironment,and actively directing effector immune responses have emerged.Notably,agents targeting DNA damage response pathways,metabolic reprogramming,as well as DLL3-targeted antibody-drug conjugates and T-cell engag-ers,have provided new opportunities to overcome immunotherapy resistance.Systematic integration of resistance mechanisms with biologically driven combination strategies may facilitate the transition of immunotherapy in SCLC toward greater precision and sustained clinical benefit.
