2005~2022年中国肿瘤医疗资源保障水平的空间差异及动态变化OA
Spatial and Temporal Variations in the Level of Cancer Medical Resource Provision in China,2005~2022
目的:揭示我国常住人口视角下 2005~2022 年肿瘤医疗资源保障水平的区域差异、空间分布以及空间集聚性,以期为优化肿瘤医疗资源配置提供科学依据.方法:在构建基于常住人口的肿瘤医疗资源保障指数基础上,运用变异系数、空间自相关、热点分析等探讨我国肿瘤医疗资源保障水平的时空格局演变特征.结果:2005~2022年我国万人常住人口拥有的肿瘤科床位数从 0.55 张增加到 1.93 张,肿瘤医疗资源保障水平总体呈现出上升趋势,区域尺度上西部地区与其他地区的差距逐渐扩大;省域尺度上保障水平的变异系数由 0.62 下降到 0.37,并保持稳定的两个阶段特征;空间自相关结果显示呈现显著空间正相关(各年 P<0.01)且 Moran's I 指数呈现出波动型上升趋势;热点分析显示我国保障水平从东部到西部由热点区向冷点区渐变式的梯度空间集聚特征.结论:尽管我国肿瘤医疗资源保障水平空间差异有所缩小,但非均衡现象依然凸显,特别是在区域层面的差距尤为严重.建议加强区域联动、建立起精准帮扶制度,充分利用互联网大数据等技术支撑远距离优质医疗资源对实际就医需求的保障,建设与常住人口分布相匹配的肿瘤监测点网络与转诊体系.
Objective:To reveal the regional differences,spatial distribution and agglomeration of the level of cancer medical resource security in China from 2005 to 2022 from the perspective of permanent population,so as to provide scientif-ic basis for optimizing the spatial allocation of cancer medical resources.Methods:A cancer medical resource guarantee index based on permanent population was constructed,and the coefficient of variation,spatial autocorrelation and hotspot a-nalysis were used to explore the evolution characteristics of the spatial-temporal pattern of cancer medical resource guarantee level in China.Results:From 2005 to 2022,the number of oncology beds per 10,000 permanent residents in China in-creased from 0.55 to 1.93.Overall,the level of cancer medical resource provision showed an upward trend,while at the re-gional scale,the gap between the western region and other regions gradually widened.At the provincial scale,the coefficient of variation for the security level decreased from 0.62 to 0.37,with the decline occurring in two stable phases.Spatial auto-correlation results indicated a significant positive spatial correlation(P<0.01 for all years),with the Moran's I index showing a fluctuating upward trend.Hot spot analysis further revealed a gradual transitional spatial clustering pattern of Chi-na's cancer medical resource provision level,shifting progressively from hot spots in the eastern region to cold spots in the western region.Conclusion:Although the spatial disparity in the level of cancer medical resource provision in China has narrowed,the imbalance remains pronounced,particularly in the gap between regions at the regional level.It is suggested to strengthen regional linkage and establish a precise assistance system.The Internet,big data,and other technologies should be fully leveraged to ensure that remote high-quality medical resources meet actual healthcare needs.Additionally,a tumor monitoring network and referral system aligned with the permanent population distribution should be established.
