视网膜母细胞瘤患者TIMP-1、HIF-1α表达及其与MMP-9、VEGF的相关性研究OA
Expressions of TIMP-1 and HIF-1α,and Their Correlation with MMP-9 and VEGF in Patients with Retinoblastoma
目的:探讨视网膜母细胞瘤(retinoblastoma,RB)患儿基质金属蛋白酶组织抑制剂-1(tissue inhibitor of met-alloproteinase-1,TIMP-1)、缺氧诱导因子-1α(hypoxia-inducible factor-1α,HIF-1α)表达水平变化,分析其与基质金属蛋白酶-9(matrix metalloproteinase-9,MMP-9)、血管内皮生长因子(vascular endothelial growth factor,VEGF)的相关性.方法:选取 2018 年 3 月至 2025 年 1 月本院 83 例 RB 患儿纳入 RB 组,纳入 30 例良性眼病患儿作为良性眼病组,另选取同期行健康体检儿童 80 例作为健康对照组,检测三组血清 TIMP-1、HIF-1α、MMP-9、VEGF 表达水平,对比三组血清指标差异,分析其表达量与 RB 患儿临床病理特征的关系,采用 ROC 曲线评估血清指标对 RB 发生的诊断价值.通过 Pearson 相关性分析各项组织样本与血清指标之间的关联.对 RB 组接受临床干预的患儿,比较其治疗前后血清 TIMP-1、HIF-1α、MMP-9、VEGF 水平差异.结果:RB 组血清 TIMP-1、HIF-1α、MMP-9 及 VEGF 水平均高于良性眼病组、健康对照组(P<0.05);不同临床特征患儿血清 TIMP-1、HIF-1α、MMP-9 及 VEGF 水平比较:国际眼内RB 分期(IIRC)D/E 期>C 期>A/B 期,未分化型>分化型,视神经侵犯特征中筛板后侵犯>筛板前侵犯>无侵犯,脉络膜侵犯特征中弥漫性侵犯>局灶性侵犯>无侵犯(P<0.05);血清 TIMP-1、HIF-1α、MMP-9 及 VEGF 联合检测评估 RB的 AUC 为 0.863,高于各项指标单独检测的 0.726、0.761、0.703、0.766(P<0.05);Pearson 相关性分析结果显示:在RB 患儿中,血清 TIMP-1 水平与 HIF-1α、VEGF、MMP-9 水平呈正相关,HIF-1α 水平与 MMP-9、VEGF 呈正相关(P<0.05);与治疗前比较,RB 组患儿治疗后血清 TIMP-1、HIF-1α、MMP-9及 VEGF 水平下降(P<0.05).结论:RB 患儿血清 TIMP-1、HIF-1α、MMP-9及 VEGF 水平均显著升高,其升高幅度与肿瘤分期、分化程度、侵袭性病理特征呈正相关,各项指标在血清学水平上存在显著的正相关关系,四项指标联合检测可提升对 RB 的鉴别诊断效能,但联合检测的特异度较低,作为独立筛查工具存在局限性,其临床价值更多体现在辅助诊断与风险分层.
Objective:To investigate the expression levels of tissue inhibitor of metalloproteinase-1(TIMP-1)and hypox-ia-inducible factor-1α(HIF-1α)in children with retinoblastoma(RB),and to analyze their respective correlations with ma-trix metalloproteinase-9(MMP-9)and vascular endothelial growth factor(VEGF).Methods:A total of 83 RB children,30 children with benign ophthalmopathy,and 80 healthy children who underwent routine physical examination were enrolled as RB group,benign ophthalmopathy group,and control group,respectively,from March 2018 to January 2025.The serum levels of TIMP-1,HIF-1α,MMP-9,and VEGF in the three groups were measured and compared.The relationships between these serum markers and clinicopathological charac-teristics of RB patients were analyzed.Receiver operating characteristic(ROC)curves were constructed to evaluate the diag-nostic value of these serum markers for predicting RB occurrence.Pearson correlation analysis was performed to examine the correlations among the serum levels of the four markers.Furthermore,in RB patients who received clinical intervention,the serum levels of TIMP-1,HIF-1α,MMP-9,and VEGF were compared before and after treatment.Results:The levels of ser-um TIMP-1,HIF-1α,MMP-9 and VEGF in RB group were higher than those in benign ophthalmopathy group and control group(P<0.05).Among children with different clinical characteristics,the serum levels of TIMP-1,HIF-1α,MMP-9,and VEGF were as follows:stage D/E>stage C>stage A/B(according to the International Intraocular Retinoblastoma Classification,IIRC);undifferentiated type>differentiated type;post-laminar invasion>pre-laminar invasion>no invasion(optic nerve invasion);and diffuse invasion>focal invasion>no invasion(choroidal invasion)(all P<0.05).The area under the ROC curve(AUC)of the combined detection of serum TIMP-1,HIF-1α,MMP-9,and VEGF for predicting RB occurrence was 0.863,which was significantly greater than that of each single marker alone(0.726,0.761,0.703,and 0.766,respectively;all P<0.05).The results of Pearson correlation analysis showed that in RB children,serum TIMP-1 level was positively correlated with HIF-1α,VEGF,and MMP-9 levels,and HIF-1α level was positively correlated with MMP-9 and VEGF levels(all P<0.05).Furthermore,compared with the levels before treatment,the serum levels of TIMP-1,HIF-1α,MMP-9,and VEGF in the RB group significantly decreased after treatment(P<0.05).Conclusion:The serum levels of TIMP-1,HIF-1α,MMP-9,and VEGF significantly increase in children with RB,and the magnitude of elevation is positively correlated with tumor staging,degree of differentiation,and invasive pathological characteristics.Sig-nificant positive correlations are observed between any pair of these serum markers.Combined detection of the four markers improves diagnostic efficacy for RB.However,the specificity of this combined approach is low,limiting its utility as an inde-pendent screening tool.Its clinical value lies primarily in its role as a diagnostic aid and in risk stratification.
