组织胞浆菌病误诊为结缔组织病原因分析并文献复习OA

Objective To investigate the causes of misdiagnosis of histoplasmosis as connective tissue disease and to conduct a literature review.Methods A retrospective analysis was performed on the clinical data of a patient with histoplasmosis misdiagnosed as connective tissue disease at the 940th Hospital of Joint Logistics Support Force of the PLA in April 2024.Results The main clinical manifestations of the patient were indurated swelling of the face and neck with fever.The patient was initially diagnosed as connective tissue disease by relevant examinations.There was no significant improvement after Glucocorticoid treatment.After admission,the relevant examinations were performed,and the pathological diagnosis was histoplasmosis.The duration of misdiagnosis was 8 months.After the diagnosis,the clinical symptoms and signs were significantly improved after regular antifungal treatment,with favorable prognosis.Conclusion Histoplasmosis has atypical clinical manifestations and is easily confused with connective tissue diseases.In patients with unexplained fever,indurated swelling in the face and/or neck,enlargement of the liver,spleen and lymph nodes,and relevant exposure history,the possibility of histoplasmosis should be highly suspected,and targeted diagnostic measures should be taken to ensure early diagnosis and reduce the rate of misdiagnosis and mistreatment.
