血清钙联合影像特征与颅内动脉瘤破裂的相关性研究OA
Correlation of serum corrected calcium combined with imaging features with the rupture of intracranial aneurysms
目的 探讨血清校正钙联合影像特征与颅内动脉瘤破裂状态的相关性.方法 回顾性纳入2023 年1 月—2025 年6 月于徐州医科大学第二附属医院神经外科确诊的 150 例颅内动脉瘤患者(71 例破裂).采用 LASSO 及多因素 Logistic 回归分析破裂的相关因素,构建联合模型(校正钙、高血压、夹层/不规则形态、最大直径、最大高度),并与单纯影像模型比较其相关性强度(AUC、NRI、IDI),时间外部验证(n=50)评估稳定性.结果 联合模型中,校正钙(OR=0.347)、高血压(OR=4.151)、夹层(OR=3.967)、不规则形态(OR=4.112)、最大直径(OR=1.480)、最大高度(OR=1.623)均与破裂相关(均P<0.05).联合模型AUC=0.855,高于单纯影像模型的 0.792(P=0.030),NRI=0.21,IDI=0.08.时间验证 AUC=0.830,校准良好.结论 血清校正钙联合多项影像特征与颅内动脉瘤破裂状态显著相关,联合模型的相关性评估效能优于单纯影像模型.
Objective To investigate the correlation of serum corrected calcium combined with imaging features with the rupture status of intracranial aneurysms.Methods A total of 150 patients(71 ruptured)diagnosed with intracranial aneurysms in Department of Neurosurgery,the Second Affiliated Hospital of Xuzhou Medical University from January 2023 to June 2025 were analyzed retrospectively.LASSO and multivariate Logistic regression were used to identify factors associated with rupture and to build a combined model(corrected calcium,hypertension,dissecting/irregular morphology,maximum diameter,maximum height).Its correlation strength(AUC,NRI,IDI)was compared with an imagingonly model,followed by temporal external validation(n=50).Results In the combined model,corrected calcium(OR=0.347),hypertension(OR=4.151),dissecting morphology(OR=3.967),irregular morphology(OR=4.112),maximum diameter(OR=1.480),and maximum height(OR=1.623)were significantly correlated with rupture(all P<0.05).The combined model achieved an AUC of 0.855,significantly higher than the imagingonly model(AUC=0.792,P=0.030),with NRI=0.21 and IDI=0.08.Temporal validation yielded an AUC of 0.830 with good calibration.Conclusions Serum corrected calcium combined with multiple imaging features is closely correlated with the rupture status of intracranial aneurysms,and the combined model outperforms the imagingonly model in evaluating this correlation.
