目的 在已知颈动脉超声(影像学参考标准)分组的基础上,比较稳定与易损斑块组间螺旋 CT特征及血清程序性细胞死亡因子4(PDCD4)、潜伏转化生长因子 β 结合蛋白-2(LTBP-2)水平的差异,并探讨这些指标与脑梗死(CI)患者颈动脉斑块稳定性(CPS)的相关性及联合预测价值.方法 选取2022 年3 月—2024 年11 月于华北医疗健康集团峰峰总医院就诊的164 例 CI 患者.依据影像学参考标准分为稳定斑块组(n=93)与易损斑块组(n=71).受试者工作特征(ROC)曲线评估螺旋 CT 联合血清 PDCD4、LTBP-2 预测 CI患者 CPS 的效能.Kappa 检验分析三者联合与影像学参考标准的一致性.结果 螺旋 CT 评估灵敏度为77.46%,特异度为76.34%;易损斑块组 PDCD4、LTBP-2 水平均高于稳定斑块组,差异有统计学意义(P<0.05);血清PDCD4、LTBP-2 单独及联合评估CI 患者CPS 的曲线下面积(AUC)分别为0.713、0.803 和0.924;螺旋 CT 联合血清 PDCD4、LTBP-2 评估 CI 患者 CPS 的灵敏度为92.96%,特异度为83.87%,与影像学参考标准的一致性较高(Kappa 值=0.756,P<0.001).结论 螺旋 CT 联合血清 PDCD4、LTBP-2 评估 CI 患者 CPS的价值与颈动脉超声具有较高的一致性.

Objective Based on carotid artery ultrasound imaging,the differences among spiral CT features and serum levels of programmed cell death factor 4(PDCD4)and latent transforming growth factor beta binding protein-2(LTBP-2)were compared between the stable and vulnerable plaque groups,and to explore the correlation of these indicators with carotid plaque stability(CPS)in patients with cerebral infarction(CI)and their combined predictive value.Methods A total of 164 CI patients admitted to Fengfeng General Hospital,North China Medical and Health Group from March 2022 to November 2024 were selected.According to the imaging reference standard,they were divided into stable plaque group(n=93)and vulnerable plaque group(n=71).Receiver operating characteristic(ROC)curve was used to evaluate the efficacy of spiral CT combined with serum PDCD4 and LTBP-2 in predicting CPS in CI patients.The Kappa test was used to analyze the consistency between the combination of the three and the imaging reference standard.Results The sensitivity and specificity of spiral CT evaluation were 77.46%and 76.34%,respectively.The levels of PDCD4 and LTBP-2 in the vulnerable plaque group were higher than those in the stable plaque group,and the differences were statistically significant(P<0.05).The areas under the curve(AUC)of serum PDCD4,LTBP-2 alone and their combination for evaluating CPS in CI patients were 0.713,0.803 and 0.924,respectively.The sensitivity and specificity of spiral CT combined with serum PDCD4 and LTBP-2 for evaluating CPS in CI patients were 92.96%and 83.87%,respectively,and the consistency with the imaging reference standard was high(Kappa value=0.756,P<0.001).Conclusions The value of spiral CT combined with serum PDCD4 and LTBP-2 in evaluating CPS in CI patients has a high consistency with carotid artery ultrasound.
