目的 基于孟德尔随机化(MR)分析探讨原发性头痛与失眠的双向因果关系.方法 采用双向双样本MR分析,利用全基因组关联研究(GWAS)汇总数据,以逆方差加权法(IVW)为主要分析方法,辅以MR-Egger回归、加权中位数法、简单众数法及加权众数法进行评估.通过Cochran's Q检验评估异质性,MR-Egger截距法和MR-PRESSO法检测多效性,留一法分析评估结果稳定性.结果 正向MR分析显示,其他头痛综合征可显著增加失眠的发病风险(OR=1.078,95%CI:1.053~1.104,P<0.001,PFDR<0.001);反向MR分析表明,失眠可显著增加其他头痛综合征的发病风险(OR=2.179,95%CI:1.448~3.279,P<0.001,PFDR=0.002),且与三叉神经痛存在潜在正向关联(OR=2.888,95%CI:1.104~7.558,P=0.031,PFDR=0.177).敏感性分析证实正向结果稳健;反向分析中,失眠与其他头痛综合征虽存在异质性和潜在多效性,但经MR-PRESSO校正后结果稳健.结论 本研究证实其他头痛综合征与失眠之间存在稳健的双向因果关联,失眠可能是三叉神经痛的潜在危险因素.

Objective To investigate the bidirectional causal relationship between primary head-ache disorders and insomnia using Mendelian randomization(MR)analysis.Methods A bidirec-tional two-sample MR analysis was performed using summary statistics from genome-wide association studies(GWAS).The inverse-variance weighted(IVW)method was used as the primary analysis,supplemented by MR-Egger regression,weighted median,simple mode,and weighted mode methods.Heterogeneity was assessed using Cochran's Q test.Pleiotropy was evaluated using the MR-Egger in-tercept and MR-PRESSO methods.The stability of the results was assessed using leave-one-out analy-sis.Results Forward MR analysis indicated that other headache syndromes significantly increased the risk of insomnia(OR=1.078,95%CI:1.053 to 1.104,P<0.001,PFDR<0.001).Reverse MR analysis demonstrated that insomnia significantly increased the risk of other headache syndromes(OR=2.179,95%CI:1.448 to 3.279,P<0.001,PFDR=0.002)and showed a potential positive as-sociation with trigeminal neuralgia(OR=2.888,95%CI:1.104 to 7.558,P=0.031,PFDR=0.177).Sensitivity analyses confirmed the robustness of the forward causal estimates.In the reverse analysis,although heterogeneity and potential pleiotropy were observed for the association between insomnia and other headache syndromes,the results remained robust after MR-PRESSO correction.Conclusion This study confirms a robust bidirectional causal association between other headache syndrome and insom-nia,suggesting that insomnia may be a potential risk factor for trigeminal neuralgia.
