肺腺癌与非腺癌脑转移影像学特征及区域分布的比较分析OA
Comparative analysis of imaging features and regional distribution of brain metastases between lung adenocarcinoma and non-adenocarcinoma
目的 基于电子计算机断层扫描(CT)/磁共振成像(MRI)分析肺腺癌与非腺癌脑转移患者的影像学特征及脑转移区域分布差异.方法 回顾性收集102例肺癌脑转移患者的病历资料,根据病理诊断结果将患者分为肺腺癌组(n=52)与非腺癌组(n=50).采用西门子双源CT及飞利浦Ingenia 3.0T MRI行头颅平扫、增强扫描及磁敏感加权成像(SWI),运用多平面重组、最大密度投影后处理技术,分析脑转移灶区域分布、强化方式及水肿程度.比较2组患者的一般资料、脑转移区域及脑转移灶特征,并将单因素分析中P<0.05的因素纳入多因素Logistic回归模型,筛选区分病理类型的独立影响因素.结果 与肺腺癌组相比,非腺癌组有吸烟史者占比更高、脑转移灶数目更多、最大转移灶直径更大,差异均有统计学意义(P<0.05).肺腺癌组脑转移灶在双侧大脑半球的分布占比高于非腺癌组,在小脑的分布占比低于非腺癌组,差异均有统计学意义(P<0.05).与非腺癌组相比,肺腺癌组的结节均匀强化特征占比更高,环形强化、不均匀斑片强化特征更低,差异有统计学意义(P<0.05);与非腺癌组相比,肺腺癌组无水肿、轻度水肿占比更高,中度、重度水肿占比更低,差异有统计学意义(P<0.05).多因素Logistic回归分析结果显示,吸烟史、脑转移灶数目、最大转移灶直径、水肿程度均为区分肺腺癌与非腺癌脑转移的独立影响因素(P<0.05).结论 肺腺癌与非腺癌脑转移的CT/MRI影像在区域分布、强化方式及水肿程度方面存在差异,吸烟史、脑转移灶数目、最大转移灶直径及水肿程度是区分二者的独立影响因素.
Objective To analyze the imaging features and regional distribution differences of brain metastases from lung adenocarcinoma and non-adenocarcinoma based on computed tomography(CT)and magnetic resonance imaging(MRI).Methods The clinical data of 102 patients with brain metastases from lung cancer were retrospectively collected.Patients were divided into lung ade-nocarcinoma group(n=52)and non-adenocarcinoma group(n=50)according to the pathological diagnosis.Non-contrast CT,contrast-enhanced CT,and susceptibility-weighted imaging(SWI)were performed using a Siemens dual-source CT scanner and a Philips Ingenia 3.0T MRI scanner.Post-processing techniques including multiplanar reconstruction and maximum intensity projection were em-ployed to analyze the regional distribution,enhancement patterns,and edema extent of brain metasta-ses.General characteristics,metastatic brain regions,and imaging features of brain metastases were compared between the two groups.Variables with P less than 0.05 in univariate analysis were includ-ed in a multivariate logistic regression model to identify independent factors distinguishing the pathologi-cal types.Results Compared with the lung adenocarcinoma group,the non-adenocarcinoma group had a significantly higher proportion of patients with a smoking history,a greater number of brain metastases lesions,and a larger maximum metastasis diameter(P<0.05).The proportion of brain metastases located in the bilateral cerebral hemispheres was significantly higher,and the proportion in the cere-bellum was significantly lower in the lung adenocarcinoma group than in the non-adenocarcinoma group(P<0.05).Compared with the non-adenocarcinoma group,the lung adenocarcinoma group had a significantly higher proportion of homogeneous nodular enhancement and significantly lower proportions of ring enhancement and heterogeneous patchy enhancement(P<0.05).The lung ade-nocarcinoma group also had significantly higher proportions of absent and mild edema and signifi-cantly lower proportions of moderate and severe edema compared with the non-adenocarcinoma group(P<0.05).Multivariate logistic regression analysis showed that smoking history,the number of brain metastases lesions,maximum metastasis diameter,and edema extent were independent factors distinguishing lung adenocarcinoma from non-adenocarcinoma brain metastases(P<0.05).Con-clusion CT and MRI imaging demonstrate differences in regional distribution,enhancement pat-terns,and edema extent between brain metastases from lung adenocarcinoma and non-adenocarcino-ma.Smoking history,number of brain metastases,maximum metastasis diameter,and edema extent are independent factors distinguishing the two pathological types.
