血清鸢尾素、新喋呤及甘油三酯葡萄糖指数在慢性心力衰竭合并房颤患者预后评估中的应用价值OA
Clinical value of serum irisin,neopterin,and triglyceride-glucose index in assessment of prognosis in patients with chronic heart failure complicated with atrial fibrillation
目的 探讨血清鸢尾素(Irisin)、新喋呤(Neopterin)及甘油三酯葡萄糖(TyG)指数在慢性心力衰竭(CHF)合并心房颤动(房颤)患者预后评估中的应用价值.方法 前瞻性选取2022年1月-2025年1月咸阳市中心医院收治的215例CHF合并房颤患者,依据纳入和排除标准最终纳入196例.根据出院后30 d内是否发生心功能失代偿再入院或心源性死亡等终点事件,将患者分为预后不良组与预后良好组.收集2组患者一般资料,比较血清Irisin、Neopterin及TyG指数水平差异.采用多因素Logistic回归分析CHF合并房颤患者预后不良的危险因素,绘制受试者工作特征(ROC)曲线评估各指标、血清联合模型(Irisin、Neopterin及TyG指数)和综合模型(总胆红素、血红蛋白、Irisin、Neopterin及TyG指数)对患者预后的预测价值.结果 196例CHF合并房颤患者中,51例(26.02%)预后不良.采用Bonferroni校正后,预后不良组总胆红素水平高于预后良好组,血红蛋白水平低于预后良好组,差异有统计学意义(P<0.002 38).采用Bonferroni校正后,与预后良好组比较,预后不良组血清Irisin水平降低,血清Neopterin水平及TyG指数升高,差异有统计学意义(P<0.016 7).多因素Logistic回归分析结果显示,总胆红素、Neopterin及TyG指数升高均为CHF合并房颤患者预后不良的危险因素(P<0.05),血清Irisin、血红蛋白水平升高则为保护因素(P<0.05).ROC曲线分析结果显示,血清Irisin、Neopterin及TyG指数单独预测CHF合并房颤患者预后不良的曲线下面积(95%CI)分别为0.760(0.694~0.818)、0.791(0.727~0.845)、0.799(0.736~0.853);血清联合模型、综合模型预测的AUC分别为0.896(0.845~0.935)、0.935(0.891~0.965);DeLong检验结果显示,血清联合模型效能高于各指标单独预测效能,但低于综合模型预测效能(P<0.05).结论 Neopterin和TyG指数升高是CHF合并房颤患者预后不良的危险因素,血清Irisin水平升高则是其保护因素.血清联合模型在单中心队列中显示出良好的预测效能.
Objective To investigate the value of serum irisin,neopterin,and triglyceride-glu-cose(TyG)index in assessment of prognosis in patients with chronic heart failure(CHF)complicat-ed with atrial fibrillation.Methods A total of 215 patients with CHF and atrial fibrillation admitted to Xianyang Central Hospital from January 2022 to January 2025 were prospectively enrolled,and 196 were finally included based on inclusion and exclusion criteria.According to the occurrence of end-point events such as readmission for cardiac decompensation or cardiac death within 30 days after discharge,the patients were divided into poor-prognosis group and good-prognosis group.General data of the two groups were collected,and differences in serum irisin,neopterin,and TyG index levels were compared.Multivariate logistic regression analysis was used to identify risk factors for poor prognosis in CHF patients with atrial fibrillation.Receiver operating characteristic(ROC)curves were plotted to evaluate the predictive value of each indicator,the combined serum model(irisin,neopterin,and TyG index),and the comprehensive model(total bilirubin,hemoglobin,irisin,neopterin,and TyG index)for patient prognosis.Results Among 196 patients with CHF and atrial fibrillation,51(26.02%)had a poor prognosis.After Bonferroni correction,the poor-prognosis group had significantly higher total bilirubin levels and lower hemoglobin levels than the good-prognosis group(P<0.002 38).After Bonferroni correction,compared with the good-progno-sis group,the poor-prognosis group showed significantly lower serum irisin levels and significantly higher serum neopterin levels and TyG index(P<0.016 7).Multivariate logistic regression analy-sis showed that elevated total bilirubin,neopterin,and TyG index were risk factors for poor progno-sis in CHF patients with atrial fibrillation(P<0.05),while elevated serum irisin and hemoglobin levels were protective factors(P<0.05).ROC curve analysis showed that the area under the curve(AUC)(95%CI)of serum irisin,neopterin,and TyG index alone for predicting poor prognosis in CHF patients with atrial fibrillation were 0.760(0.694-0.818),0.791(0.727-0.845),and 0.799(0.736-0.853),respectively;the AUCs of the combined serum model and the comprehen-sive model were 0.896(0.845-0.935)and 0.935(0.891-0.965),respectively.DeLong test results showed that the performance of the combined serum model was superior to that of each indi-vidual indicator,but inferior to that of the comprehensive model(P<0.05).Conclusion Elevat-ed neopterin and TyG index are risk factors for poor prognosis in CHF patients with atrial fibrillation,while elevated serum irisin is a protective factor.The combined serum model shows good predictive efficacy in this single-center cohort.
