基于"浊邪害清"理论探析良性前列腺增生的辨治OA

Benign prostatic hyperplasia is one of the common and frequently occurring diseases in urology,which has the characteristics of slow onset,prolonged course,persistence and difficulty in recovery.Ancient and modern physicians have classified it under the categories of"dribbling and retention of urine","prostatic hypertrophy"and"urinary stoppage"according to varying symptoms in traditional Chinese medicine.Based on the theory of"turbid pathogens invading the clear orifice",it is believed that"failure of clear yang to ascend,stagnation of turbid pathogens"as the initiating pathological factor runs through its occurrence and development,and"turbid pathogens invading clear orifice,dysfunction of essence chamber"is viewed as its core pathogenesis.Besides,its basic pathogeneses involve:kidney deficiency and turbidity exuberance,imbalance of yin-yang;dysfunction of the lung and spleen,stagnation of turbid qi;accumulation of damp-heat,turbidity blocking the seminal orifice;and phlegm-stasis-turbidity entanglement,blockage of the urinary tract.Guided by the pathogenesis of"turbid pathogens invading the clear orifice",targeted treatment principles are proposed:nourishing yin and warming yang to disperse turbid pathogens;regulating the lung and spleen function to ascend clear qi and descend turbid qi;clearing damp-heat to eliminate turbidity and unblock the orifice;dispelling phlegm and resolving stasis to eliminate turbidity and dredge collaterals.Ultimately,the accompanying symptoms are supposed to be improved by restoring the normal physiological function of the seminal chamber.This study is aimed at providing a new perspective and reference for the syndrome differentiation and treatment of this disease in traditional Chinese medicine.
