基于自噬及氧化应激探讨"益肾通络法"的微观机制及在糖尿病肾病中的作用OA
Discussion on the micro-mechanism of the"kidney-enhancing and collateral-promoting method"based on autophagy and oxidative stress and its effect in the treatment of diabetic kidney disease
糖尿病肾病的中医基本病机为"肾虚络阻",基于此的"益肾通络法"是其基本治法,但是目前对于"益肾通络法"治疗糖尿病肾病的微观机制尚不清晰.促氧化物质与抗氧化物质失衡引起的氧化应激损伤是糖尿病肾病发生发展的重要机制之一,而自噬通路激活则是有效对抗肾脏固有细胞氧化应激损伤的关键所在."益肾通络法"能够通过激活自噬通路,进而清除糖尿病肾病肾脏固有细胞过度堆积的促氧化物质,从而恢复氧化/抗氧化平衡,对抗细胞氧化应激损伤.团队在前期研究的基础上提出,激活自噬通路对抗氧化应激损伤为"益肾通络法"治疗糖尿病肾病的微观机制,阐明其内在机制将为中医药治疗糖尿病肾病提供理论依据,传承精华,守正创新.
The basic pathogenesis of diabetic kidney disease(DKD)is identified as"kidney deficiency and collateral obstruction"in traditional Chinese medicine,based on which the primary method for its treatment features"enhancing the kidney and promoting kidney collaterals",but the underlying microscopic mechanisms of the method still remain unclear.Oxidative stress injury caused by imbalance between pro-oxidant and antioxidant is one of the important pathogeneses of DKD,and autophagy pathway activation is the key to effectively resist oxidative stress injury of renal intrinsic cells.The"kidney-enhancing and collateral-promoting method"can activate the autophagy pathway,and then remove the excessive accumulation of pro-oxidation substances in the intrinsic cells of DKD kidneys,thus restoring the balance of oxidation/antioxidation and resisting the oxidative stress damage of cells.On the basis of previous research,it is proposed that activating the autophagy pathway to counteract oxidative stress damage is the micro-mechanism of the"kidney-enhancing and collateral-promoting method"for treating DKD,and that elucidating its intrinsic mechanism will provide a theoretical basis for the treatment of DKD in traditional Chinese medicine,inheriting the essence and innovating with integrity.
