口服孕激素与左炔诺孕酮宫内缓释系统放置治疗对子宫内膜息肉患者术后性激素、子宫内膜厚度及复发的影响OA
Effect of oral progesterone and levonorgestrel intrauterine sustained-re-lease system placement therapy on postoperative sex hormones,endome-trial thickness,and recurrence in patients with endometrial polyps
目的 探讨口服孕激素与左炔诺孕酮宫内缓释系统放置治疗对子宫内膜息肉患者宫腔镜术后性激素、子宫内膜厚度及复发的影响.方法 选取2023年1月至2025年1月九江市妇幼保健院收治的96例子宫内膜息肉患者作为研究对象,按照随机数字表法分为观察组和对照组,每组48例.对照组术后第14天口服地屈孕酮片治疗,观察组术后宫内放置左炔诺孕酮宫内缓释系统治疗.比较两组患者性激素、子宫内膜厚度、不良反应及复发情况.结果 两组患者治疗前后性激素水平比较,差异无统计学意义(P＞0.05).观察组治疗后子宫内膜厚度低于对照组,差异有统计学意义(P＜0.05).观察组不良反应总发生率及复发率均低于对照组,差异有统计学意义(P＜0.05).结论 子宫内膜息肉患者宫腔镜术后宫腔内放置左炔诺孕酮宫内缓释系统的效果优于口服孕激素,能够更好地改善子宫内膜厚度,降低复发率,且安全性高,对性激素水平影响小,值得临床推广应用.
Objective To explore the effect of oral progesterone and the placement of levonorgestrel intrauterine sus-tained-release system on the sex hormones,endometrial thickness,and recurrence in patients with endometrial polyps after hysteroscopic surgery.Methods A total of 96 patients with endometrial polyps admitted to Jiujiang Maternal and Child Health Hospital were selected as the research subjects from January 2023 to January 2025,they were divided into the observation group and the control group according to the random number table method,with 48 cases in each group.The control group received oral Desogestrel Tablets on the 14th day after surgery,while the observation group received the levonorgestrel intrauterine sustained-release system placement after surgery.The sex hormones,endometrial thickness,adverse reactions,and recurrence of the two groups were compared.Results There were no statistically significant differ-ences in sex hormone levels before and after treatment between the two groups(P＞0.05).The endometrial thickness of the observation group was lower than that of the control group after treatment,and the difference was statistically significant(P＜0.05).The total incidence of adverse reactions and recurrence rate in the observation group were lower than those in the control group,and the differences were statistically significant(P＜0.05).Conclusion The effect of levonorgestrel intrauter-ine sustained-release system placement in the uterine cavity after hysteroscopy for patients with endometrial polyps is better than oral progesterone.It can better improve endometrial thickness,reduce recurrence rate,and has high safety,with little impact on sex hormone levels,and is worthy of clinical promotion and application.
