益气健脾法联合快速康复治疗对大肠癌患者术后康复的影响OA
Effects of nourishing qi to invigorate spleen combined with enhanced recovery after surgery on postoperative rehabilitation in patients with colorectal cancer
目的 观察益气健脾法联合快速康复治疗对大肠癌患者术后康复的影响.方法 将140 例大肠癌术后脾虚型患者按照随机数字表法分为 2 组,对照组 70 例术后予快速康复治疗,治疗组 70 例在对照组治疗基础上予益气健脾法治疗.2 组均治疗21 d.记录2 组消化道功能恢复情况,比较2 组治疗前后中医证候评分、免疫功能指标(CD3+、CD4+及 CD4+/CD8+)、皮质醇(Cor)、C 反应蛋白(CRP)及营养状况[白蛋白(ALB)、前白蛋白(PAB)].结果 2 组治疗后中医证候各项评分及总分均较本组治疗前降低(P<0.05),且治疗组治疗后均低于对照组(P<0.05).治疗组治疗后消化道功能恢复情况指标肠鸣音恢复时间、肛门排气时间及首次排便时间均短于对照组(P<0.05).2 组治疗后外周血 T 淋巴细胞亚群 CD3+、CD4+占比及 CD4+/CD8+值均较本组治疗前升高(P<0.05),且治疗组治疗后均高于对照组(P<0.05).2组治疗后Cor、CRP 水平均较本组治疗前降低(P<0.05),且治疗组治疗后均低于对照组(P<0.05).2 组治疗后ALB、PAB水平均较本组治疗前升高(P<0.05),且治疗组治疗后均高于对照组(P<0.05).结论 益气健脾法联合快速康复治疗有助于缓解大肠癌术后脾虚型患者的临床症状,加快消化道功能的恢复,提高免疫功能,减轻应激反应,提高患者营养水平,从而加速患者康复.
Objective To observe the effects of nourishing qi to invigorate spleen combined with enhanced recovery after surgery(ERAS)on postoperative rehabilitation in patients with colorectal cancer.Methods A total of 140 postoperative patients with colorectal cancer and spleen deficiency syndrome were randomly assigned to two groups using a random number table.The control group(n=70)received ERAS after surgery,while the treatment group(n=70)received nourishing qi to invigorate spleen in addition to the same ERAS regimen.Both groups were treated for 21 d.Gastrointestinal function recovery was recorded in both groups.Before and after treatment,traditional Chinese medicine(TCM)syndrome scores,immune function indices(CD3+,CD4+,and CD4+/CD8+),cortisol(Cor),C-reactive protein(CRP),and nutritional status(albumin[ALB],prealbumin[PAB])were compared.Results After treatment,all TCM syndrome scores and the total score decreased in both groups compared with pre-treatment values(P<0.05),and were lower in the treatment group than in the control group(P<0.05).In the treatment group,recovery time of bowel sounds,first flatus time,and first defecation time were shorter than those in the control group(P<0.05).After treatment,the proportions of peripheral blood T lymphocyte subsets CD3+and CD4+,as well as the CD4+/CD8+ratio,increased in both groups compared with pre-treatment values(P<0.05),and were higher in the treatment group than in the control group(P<0.05).After treatment,Cor and CRP levels decreased in both groups compared with pre-treatment values(P<0.05),and were lower in the treatment group than in the control group(P<0.05).After treatment,ALB and PAB levels increased in both groups compared with pre-treatment values(P<0.05),and were higher in the treatment group than in the control group(P<0.05).Conclusion Nourishing qi to invigorate spleen combined with ERAS can alleviate clinical symptoms in postoperative colorectal cancer patients with spleen deficiency syndrome,accelerate gastrointestinal function recovery,improve immune function,reduce stress responses,enhance nutritional status,and thereby promote postoperative rehabilitation.
