饮食模式、饮食动机与营养状况对老年人骨骼肌质量影响的横断面调查研究OA
Associations of dietary patterns,dietary motivations,and nutritional status with skeletal muscle mass in Chinese older adults:A structural equation modeling study
目的 基于自我决定理论,探讨饮食模式、饮食动机及营养状况对老年人骨骼肌质量的影响.方法 采用横断面研究设计.于2023年4月至12月,通过便利抽样法选取湖南省某郊区和农村地区的249名老年人为调查对象.采用饮食动机量表、微型营养评估简表及中文版食物频率问卷收集数据.身体成分通过生物电阻抗分析法进行评估,骨骼肌质量采用相对骨骼肌质量指数进行标准化.采用主成分分析提取饮食模式,并运用结构方程模型检验各变量间的路径关系.结果 共提取出两种饮食模式.饮食动机受性别与教育背景影响,并对骨骼肌质量(β=0.162,P<0.001)、营养状况(β=0.278,P<0.001)及饮食模式(模式1:β=0.124,P=0.050;模式2:β=0.272,P<0.001)均有显著的直接影响.两种饮食模式均可直接作用于骨骼肌质量(模式1:β=0.338,P<0.001;模式2:β=0.430,P<0.001).此外,饮食模式直接影响营养状况(模式1:β=0.064,P<0.001;模式2:β=0.649,P<0.001),而营养状况对骨骼肌质量具有显著直接效应(β=0.177,P=0.008),提示饮食模式可通过营养状况间接作用于骨骼肌质量.结论 饮食动机可直接影响饮食模式,两者均可直接或通过改善营养状况间接影响骨骼肌质量,提示医护人员应基于老年人个体饮食动机制订针对性干预策略,以增强健康饮食模式的依从性,促进骨骼肌健康.
Objectives:This study aimed to explore the relationships between dietary patterns,motivation,nutri-tional status,and skeletal muscle mass guided by the Self-Determination Theory. Methods:This study employed a cross-sectional design.We invited 249 older adults using the conve-nience sampling method from suburban and rural areas of Hunan Province from April to December 2023.Data were collected using the Eating Motivation Survey(TEMS),the Mini Nutritional Assessment-Short Form(MNA-SF),and the Chinese version of the Food Frequency Questionnaire(FFQ).Bioelectrical Impedance Analysis was used to assess body composition,with skeletal muscle mass standardized using the Relative Skeletal Muscle Index(RSMI).Principal component analysis(PCA)was employed to extract dietary patterns,and structural equation modeling(SEM)was used to assess interaction paths. Results:Two distinct dietary patterns emerged.Dietary motivation-shaped by gender and educational background-exhibited significant direct effects on skeletal muscle mass(β=0.162,P<0.001),nutri-tional status(β=0.278,P<0.001),and dietary patterns(pattern 1:β=0.124,P=0.050;pattern 2:β=0.272,P<0.001).Dietary patterns also had direct effects skeletal muscle mass(pattern 1:β=0.338,P<0.001;pattern 2:β=0.430,P<0.001).Moreover,dietary patterns directly affect nutritional status(pattern 1:β=0.064,P<0.001;pattern 2:β=0.649,P<0.001),and nutritional status exerted a significant direct effect on skeletal muscle mass(β=0.177,P=0.008),supporting a further indirect pathway from dietary patterns to skeletal muscle mass via nutritional status. Conclusions:Dietary motivation can directly influence dietary patterns.Both dietary motivation and dietary patterns can directly affect skeletal muscle mass and also have indirect effects by improving nutritional status.These findings underscore the necessity for tailored,motivation-based interventions in older adults to enhance dietary adherence and muscle health outcomes.
