温胞止痛饮联合董氏奇穴治疗寒凝血瘀型原发性痛经随机对照研究OA

Objective To evaluate the efficacy and safety of Wenbao Zhitong Decoction(WBZTD)combined with Dong's Acupoint Therapy in the treatment of primary dysmenorrhea(PD)patients with cold congealing and blood stasis pattern.Methods A total of 113 PD patients with cold congealing and blood stasis pattern who visited Gynecology Outpatient Clinics,First Affiliated Hospital,Heilongjiang University of Chinese Medicine from August 2023 to August 2024 were screened.Among them,90 cases were included and randomly assigned to three groups via a random number table method:the Chinese herb group(treated with WBZTD alone),the acupuncture group(treated with Dong's Acupoint Therapy alone),and the combination group(treated with WBZTD plus Dong's Acupoint Therapy),with 30 cases in each group.All three groups received three menstrual cycles.The total effective rate,Chinese medicine(CM)pattern scores,Visual Analog Scale(VAS)scores,Cox Menstrual Symptom Scale(CMSS)scores,serum levels of prostaglandin E2(PGE2)and cyclooxygenase-2(COX-2)were analyzed after treatment.A three-month follow-up was performed.Results One patient in the combination group dropped out,and 89 patients completed the study.Compared with baseline values before treatment,all three groups showed significant reductions in CM pattern,VAS,and CMSS scores after treatment(P＜0.05,P＜0.01).The combination group exhibited significantly lower scores than the Chinese herbal medicine group and the acupuncture group both after treatment and at follow-up(P＜0.05,P＜0.01).The total effective rate in the combination group was 96.55％(28/29),which was significantly higher than that in the Chinese herbal medicine group(73.33％,22/30)and the acupuncture group(63.33％,19/30),the combined group showed statistically significant differences compared with the Chinese herbal medicine group and the acupuncture group,respectively(P＜0.05,P＜0.01).Compared with before treatment in the same group,the combination group and the Chinese herbal medicine group showed increased serum PGE2 levels and decreased COX-2 levels after treatment(P＜0.01),and the improvements in the combination group were superior to those in the Chinese herbal medicine group and the acupuncture group(P＜0.05,P＜0.01).All three groups showed mild adverse reactions,and no serious adverse events occurred.Conclusion Wenbao Zhitong Decoction combined with Dong's Acupoint Therapy in the treatment of PD patients with cold congealing and blood stasis pattern reduces CM pattern scores,alleviates pain intensity,shortens the duration of dysmenorrhea,modulates serum markers expressions,showing better efficacy than using Chinese herbal medicine group or acupuncture alone.
